DAMAGE WAS NOTED TO THE TAIL BOOM LH SKIN (P/N: L535A1601107) IN THE FORM OF CHAFE IN 2 LOCATIONS THAT ARE NEXT TO EACH OTHER. THE FIRST PART OF THE DAMAGE IS LOCATED AT BOTTOM SIDE OF THE TAIL BOOM WHERE BOTH LH AND RH SKIN JOIN NEAR HORIZONTAL STABILIZER LOCATION. THE DAMAGE MEASURED TO BE APPROXIMATELY 1.6” IN LENGTH AND 0.016” (APPROX. 2 PLIES) IN DEPTH. THE SECOND AREA MEASURED 1.25” IN LENGTH AND .0157” DEEP.

THESE ARE COMMON CRACKS AND WERE FOUND ON A RECEIVING INSPECTION OF THIS AIRCRAFT. *** THIS AIRCRAFT REMAINS OUT OF SERVICE AT THE TIME OF THIS SUBMISSION FOR HEAVY MAINTENANCE, AND TO REPAIR THESE CRACKS. ***

DURING CRUISE FLIGHT, THE PILOT WAS ALERTED TO A MAIN TRANSMISSION CHIP LIGHT. HE IMMEDIATELY DIVERTED BACK TO THE BASE OF OPERATIONS APPROX 3NM AWAY WITH NO ISSUES. INSPECTED THE CHIP DETECTORS. A CLASS C CHIP WAS FOUND. THE DETECTOR WAS CLEANED, SYS OIL WAS DRAINED, FILTER CHANGED & SYS OIL SERVICED. A FIFTEEN MINUTE PENALTY RUN COMPLETED THE MAINTENANCE ACTION FOR THIS EVENT WITH NO DEFECTS NOTED.

AFTER DEPARTING HOME BASE, SMELLED FUEL. STATED IT WAS STRONG. MADE A PRECAUTIONARY LANDING. MECHANIC INSPECTED AND FOUND NOTHING. ACFT RETURNED TO SERVICE. MECHANIC OPENED ACFT INTERIOR AND CHECKED THE EXPANSION TANKS FOR LEAKS, INSPECTED THE FUEL CAP, VENT LINES, AND BELLY OF THE ACFT WITH NO DISCREPANCIES NOTED. HAD THE PILOT FLY A TEST FLIGHT WITH THE AIR CONDITIONING AND FLIGHT ATTITUDE AS IT WAS WHEN THE ODOR WAS DETECTED. NO DEFECTS NOTED. ACFT RETURNED TO SERVICE.

MAIN ROTOR MAST NUT TORQUE CHECK WAS PERFORMED . THE MAST NUT WAS TO BE FOUND LOOSE. THE ROTOR HUB SHAFT WAS REMOVED AND THE SPACER TUBE WAS INSPECTED. THE PROJECTIONS ON THE SPACER TUBE WERE WORN BEYOND ALLOWABLE LIMITS.
