WHILE ENROUTE TO GC468 ON HAA TASKING CREW STATED THAT THEY HAD A STRONG INDICATION OF FUEL SMELL. THE ODOR INCREASED AND CREW STATED THAT THEY WERE HAVING NEGATIVE EFFECTS ON VISION, NAUSEA AND HEADACHES. VISUAL OBSERVATION OF LEAKING FUEL UNDER MEDIC SEAT AFT OF LITTER. DECISION WAS MADE TO CANCEL MISSION AND RTB. CONTACTED ATC FOR DIRECT BACK TO KGAO, CONTACTED OCC TO LET THEM KNOW WHAT WAS HAPPENING. FURTHER OBSERVATION NOTED FUEL MOVING FORWARD TOWARDS COCKPIT. COMPLETED EMERGENCY PROCEDURE PER THE RFM. OPENING OF THE COCKPIT WINDOWS BROUGHT A MUCH STRONGER PRESENCE OF FUMES FORWARD SO WE CLOSED THEM UNTIL OUR DESCENT. ONCE WE WERE CONFIDENT THAT AN IFR APPROACH WAS NOT NEEDED AT KGAO WE CANCELLED IFR AND DESCENDED TO 1000', SLOWED TO 80. AFTER CABIN SECURITY CHECK WAS COMPLETED, OPENED UPPER CABIN SLIDING DOOR AND COPILOT WINDOW TO IMPROVE AIRFLOW IN CABIN AND COCKPIT. LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INVESTIGATION FOUND FUEL LEAK IN LEFT SIDE OF CABIN IVO MID SPONSON, CAUSE STILL UNDER INVESTIGATION.

WHILE IN A HOVER PREPARING / BRIEFING TO HOIST RS TO THE WATER FOR TRAINING THE #1 AC GENERATOR FAIL CAUTION LIGHT ILLUMINATED. ALL BRIEFING WAS HALTED AND THE EMERGENCY PROCEDURE CHECKLIST WAS REFERENCED FOR THE CAUTION LIGHT. THE CHECKLIST CALLED FOR A RESET OF THE GENERATOR AND THEN TURNING THE GENERATOR BACK TO ON. THAT WAS ACCOMPLISHED WHICH EXTINGUISHED THE LIGHT. THE LIGHT THEN ILLUMINATED A SECOND TIME ABOUT 10 SECONDS LATER. THE APU WAS STARTED IAW WITH THE EMERGENCY OPERATING PROCEDURE CHECKLIST AND THE AIRCRAFT WAS PREPARED FOR FORWARD FLIGHT IAW THE SAR CLEANUP (SSCL) CHECKLIST TO RETURN TO BASE. AIRCRAFT RETURNED TO BASE WITHOUT INCIDENT.

LIGHT EXTINGUISHED ON IT'S OWN, CAME BACK ON DURING DOWNWIND APPROACH AND FINAL. EXTINGUISHED AGAIN AND STAYED OFF DURING LANDING AND TAXI TO RAMP. MAINTENANCE INSPECTED NR 2 FIRE/FLAME DETECTORS IAW MM 26-11-210-001. CLEANED LENS & PERFORMED FIRE DETECTION TEST WITH OVERHEAD TEST SWITCH. COULD NOT DUPLICATE, SYSTEM CHECKS GOOD. ACFT RETURNED TO SERVICE.

ON SHORT FINAL, NR 2 ENGINE FIRE LIGHT FLICKERED ON AND OFF SEVERAL TIMES, SOUNDING THE AURAL TONE. ALL OTHER INDICATIONS WERE NORMAL. MX PERSONNEL WERE UNABLE TO DUPLICATE THE PROBLEM ON THE GROUND. THE INBD AND OTBD FIRE DETECTORS AND CONNECTORS WERE CLEANED. A TEST WAS PERFORMED IAW THE MM AND NO DEFECTS WERE FOUND. THE ACFT WAS SUBSEQUENTLY FLOWN IN AN ATTMPT TO RE-CREATE THE INDICATION WITH NO FURTHER PROBLEMS NOTED.

INSTALLED DUAL HOIST ON ACFT. MX WAS NOTIFIED THRU A COMPANY SAFETY REPORT OF A POTENTIAL PROBLEM WITH A FRAME ASSY ON THE DUAL HOIST. MX CLEANED THE EFFECTED AREAS FOR INSP, AND FOUND CRACKS PROPAGATING LOGITUDINALLY AT THE UPPER AND LOWER ATTACHMENTS AND MIDDLE OF THE FRAME ITSELF. SUBSEQUENTLY THE ACFT WAS TAKEN OUT OF SERVICE AND THE HOIST WAS REMOVED.
