WHILE DOING A TR TRACK AND BALANCE FLIGHT DURING LANDING WE NOTICED THE PEDALS WERE STUCK. WE WENT AROUND TO ASSESS THE PROBLEM.

ON CLIMB OUT FROM OGLE AIRPORT, AIRCRAFT EXPERIENCED A SUDDEN MOMENTARY AND SHARP VIBRATION WITH A MOMENTARY NUMBER 2 ENGINE OUT WARNING LIGHT. EVENT LASTED LESS THAN A SECOND. AIRCRAFT FLEW BACK TO BASE FOR A PRECAUTIONARY RUN-ON LANDING. MAINTENANCE INFORMED. INVESTIGATION ONGOING
AFTER LEVELING OFF AT 5000 FT THE NO 1 FIRE DET LIGHT IS ILLUMINATED. WE VERIFIED THERE WERE NO INSTRUMENT INDICATIONS OF A FIRE IN THE NO 1 ENGINE AND CONSULTED THE QRH. AFTER NOTING THERE WAS NO IMMEDIATE DANGER WE TURNED THE AIRCRAFT TO OGLE AND COMPLETED A NORMAL LANDING WITHOUT INCIDENT
ON CLIMB OUT FROM ARA THERE WAS A STRONG SMELL OF FUEL. ALL CREW MEMBERS SMELLED IT. RTB AND SHUT DOWN.
AFTER TAKEOFF, IN CRUISE, THE NR 1 HYDRAULICS INDICATOR DROPPED TO ZERO AND THE NR 1 SERVO LIGHT IN THE CAUTION ADVISORY PANEL ILLUMINATED BY SLIGHT FEED BACK IN THE CONTROLS. AFTER REFERENCING THE EMERGENCY CHECKLIST AND ACCOMPLISHING THE TASKS, DECLARED AN EMERGENCY AND DIVERTED AND SHUT THE AIRCRAFT DOWN. 10/4 PUMP FAILURE OR LOSS OF PRESSURE.
WHILE IN A HOVER PREPARING / BRIEFING TO HOIST RS TO THE WATER FOR TRAINING THE #1 AC GENERATOR FAIL CAUTION LIGHT ILLUMINATED. ALL BRIEFING WAS HALTED AND THE EMERGENCY PROCEDURE CHECKLIST WAS REFERENCED FOR THE CAUTION LIGHT. THE CHECKLIST CALLED FOR A RESET OF THE GENERATOR AND THEN TURNING THE GENERATOR BACK TO ON. THAT WAS ACCOMPLISHED WHICH EXTINGUISHED THE LIGHT. THE LIGHT THEN ILLUMINATED A SECOND TIME ABOUT 10 SECONDS LATER. THE APU WAS STARTED IAW WITH THE EMERGENCY OPERATING PROCEDURE CHECKLIST AND THE AIRCRAFT WAS PREPARED FOR FORWARD FLIGHT IAW THE SAR CLEANUP (SSCL) CHECKLIST TO RETURN TO BASE. AIRCRAFT RETURNED TO BASE WITHOUT INCIDENT.
APPROXIMATELY 30 MINUTES AFTER RIG DEPARTURE(WR508/PONTUS)WITH PATIENT ONBOARD, #1 GENERATOR FAILED CAUTION LIGHT WITH ASSOCIATED MASTER CAUTION LIGHT ILLUMINATED. UNABLE TO RESET, DIVERTED TO KGAO (ORIGINAL DESTINATION KMSY WITH AMBULANCE TRANSPORT FOR PATIENT TO HOSPITAL). PATIENT TRANSFERRED TO DUTY AIRCRAFT FOR FURTHER TRANSPORTATION. PERFORMED ITEMS FROM EMERGENCY CHECKLIST, UNABLE TO RESET GENERATOR.
THE CABIN HEATER WAS ON AND AS USUAL SMALL AMOUNT OF SMOKE & ENGINE CLEANER ODOR IS PRESENT WHEN HEATER IS INITIALLY TURNED ON. CREW INFORMED PASSENGERS THAT THIS WAS NORMAL. DURING TAKEOFF THE AFT BAGGAGE COMPARTMENT LIGHT ILLUMINATED. TAKEOFF WAS ABORTED , RETURNED TO BASE & LANDED. HEATER WAS TURNED OFF.VISUALLY INSPECTED BAGGAGE COMPARTMENT FOUND NO EVIDENCE OF SMOKE OR FIRE. MAINTENANCE LATER FOUND CRACK IN BAGGAGE COMP. HEATER DUCT WHICH WAS REPAIRED.
DURING 750HR INSP 3 CRACKS WERE NOTED IN AFT CABIN BULKHEAD AREA. THE AREA IS BEING EVALUATED BY MAINT PERS. AND ASSISTANCE FROM MANUFACTURER FIELD REP HAS BEEN REQUESTED.
OIL PRESSURE ON #2 ENGINE FLUCTUATING BETWEEN 30-45PSI , OIL TEMP BEGAN TO DECREASE FROM 74 DEG C - 67 DEG C . OIL PRESSURE LIGHT ILLUMINATED . ENGINE WAS SECURED. CREW DIVERTED TO 2LS0.
DURING CORROSION INSP CORROSION DAMAGE NOTED TO TAIL BOOM ASSY. OEM REPAIR IAW RS139-5352 ISSUE A.
DURING 10YR INSP LOWER PANEL ASSY WAS FOUND TO HAVE CORROSION DAMAGE TO FOUR (4) AREAS. OEM REPAIR: RS139-5336-0241 ISSUE A.
DURING 50HR INSP OUTBOARD TRIM TAB WAS FOUND TO BE DE-BONDED.
BAGGAGE FLOOR PANEL WAS FOUND TO HAVE CORROSION & DE-BONDING DAMAGE.
DURING 1200HR INSPUPPER DECK ASSY WAS FOUND TO BE CORRODED & DE-BONDED
#2 FIRE DETECTOR LIGHT WITH AUDIO ILLUMINATED IN FLIGHT.NO SECONDARY INDICATION OF FIRE DETECTED. MAINT. CLEANED FIRE DETECTOR CONNECTOR PLUGS OPS CHECK OKAY.
DURING 1200HR INSP CRACKS WERE FOUND ON TAILPLANE ASSY. OEM REPAIR: RS139-5511-0365 REV A.
DURING CRUISE FLIGHT #2 ENGINE FIRE WARNING LIGHT ILLUMINATED WITH AUDIO. NO INDICATION OF SMOKE PRESENT , ALL INSTRUMENTS WERE INDICATING NORMAL. RTB TO GNO. MAINT R&R FLAME DETECTOR
DURING CRUISE PORTION OF FLIGHT, #2 ENGINE FIRE WARNING LIGHT ILLUMINATED. NO FURTHER INDICATIONS OF FIRE NOTED, CREW ELECTED TO ABORT MISSION AND RTB. MAINTENANCE CREW CLEANED CANNON PLUGS , OPS CHECK OK.NO FURTHER PROBLEMS NOTED.
DURING ENGINE SHUTDOWN PILOT MOVED THROTTLE TO CUTOFF AND THERE WAS NO RESPONSE. SECOND ATTEMPT WAS MADE WITH SAME RESPONSE. PILOT NOTED NG SLOWLY INCREASED TO 63% AND IMMEDIATELY SHUTDOWN ENGINE USING FUEL SHUTOFF VALVE SWITCH. MAINT NOTED THAT THROTTLE LINKAGE HAD BROKEN.
DURING DAILY SERVICE CHECK MAIN ROTOR RETENTION BEARING SEPARATION WAS NOTED. UPON DISASSEMBLY OF SPINDLE IT WAS NOTED THAT CONTACT HAD BEEN MADE WITH MAIN ROTOR HUB AND SPINDLE NUT
DURING A DAILY INSPECTION, THE LT POSITION LIGHT LENS WAS FOUND TO BE MISSING.
IN-FLIGHT SEPARATION OF THE RT POSITION LIGHT COVER.
RIGHT POSITION LIGHT COVER WAS DISCOVERED MISSING, DURING POST FLIGHT WALK AROUND.
POSITION LIGHT COVER WAS DISCOVERED MISSING DURING POST FLIGHT WALK AROUND.
FUEL WAS MISTING INTO COCKPIT THROUGH THE HEATER OUTLET.
MAIN ROTOR BLADE SKIN SEPARATED IN FLIGHT. MISSING SKIN SECTION IS LOCATED 61 INCHES FROM BLADE ROOT. IT IS APPROX. 27 INCHES LONG AND 13.5 INCHES WIDE. NO OTHER COMPONENTS WERE DAMAGED.
MISSING MAIN ROTOR BLADE TRIM TAB, DISCOVERED DURING POST FLIGHT INSPECTION.
DURING CRUISE FLIGHT CO-PILOT'S DOOR WINDOW SEPARATED AND DEPARTED FROM AIRCRAFT.
DURING LANDING, ENGINE CHIP LIGHT ILUMINATED, FOLLOWED BY A GRINDING NOISE THEN LOSS OF OIL PRESSURE. NR 8 BEARING FOUND DAMAGED.
AFT LOWER FUSELAGE HAS CORROSION. PERFORMED A MAJOR REPAIR.
L/H LOWER FUSELAGE PANEL CORRODED. MAJOR PERFORMED.
AFT UPPER DECK CORRODED. REPLACED THE DECK PLATE
FWD UPPER DECK PANEL CORRODED. OPEN
DURING DAILY INSPECTION. THREE TAIL ROTOR BLADES WERE FOUND TO HAVE CRACKS ADJACENT TO THE EROSION SHIELDS.
DURING MAINTENANCE, THE FORWARD LOWER PANEL WAS FOUND TO BE CORRODED BETWEEN BS 3120 AND 3900. MAJOR REPAIR PERFORMED.
DURING DAILY INSPECTION, THREE TAIL ROTOR BLADES WERE FOUND TO HAVE CRACKS NEAR THE EROSION SHIELD ,

DURING 500 HR INSPECTION, THE TAIL ROTOR BLADE WAS FOUND CRACKED NEAR THE EROSION SHIELD.
CRACK FOUND DURING ROUTINE INSPECTION. LOCATION IS NOSE BULKHEAD BS 1500. A PREVIOUS REPAIR HAD BEEN MADE TO THE AREA.
DURING CLIMB FROM FCN, ENGINE CHIP LIGHT ILLUMINATED. AIRCRAFT RETURNED TO FCN. METAL CHIPS WERE FOUND ON PLUG. REPLACED ENGINE.
RADOME HAS MULTIPLE AREAS OF CORROSION ON LT AND RT SIDES OF RADOME. A MAJOR REPAIR WAS PERFORMED.
DURING CRUISE MODE OF FLIGHT, PILOT NOTICED ENGINE OIL PRESSURE INDICATED "0". ALL OTHER INDICATIONS WERE NORMAL. PILOTED ELECTED TO RETURN TO BASE.
ON APPROACH TO FCN, NG INDICATOR STARTED TO FLUCTUATE FROM ZERO TO FULL SCALE ON THE INDICATOR. ALL OTHER READINGS WERE NORMAL. PILOT ELECTED TO RETURN TO GNO FOR MAINT.
A NINE INCH LONG CRACK ON MAIN ROTOR BLADE BETWEEN STATIONS 166 - 173.
ON CLIMBOUT FROM GNO CREW NOTICED SMOKE IN COCKPIT AND AFT BAGGAGE COMP SMOKE LIGHT ILLUMINATED. AIRCRAFT RETURNED TO BASE, UPON LANDING SMOKE WAS VISIBLE IN BAGGAGE COMPARTMENT AND A STRONG SMELL OF SMOKE WAS EVIDENT.
DURING ROUTINE INSPECTION NUMBER 1 TAIL ROTOR DRIVESHAFT HANGAR BEARING MOUNT WAS FOUND TO BE CRACKED ON INSIDE OF TAILBOOM, LEFT SIDE. OPEN
TRANSMISSION OIL PRESSURE DROPPED TO ZERO DURING FLIGHT FROM OFFSHORE PLATFORM.
CRACK FOUND IN AREA OF NR 1 TAIL ROTOR DRIVESHAFT HANGAR BEARING MOUNT ON L/H SIDE OF TAILBOOM.AWAITING FOR REPAIR FROM MANUFACTURER
