ATS DISCONNECT W/THROTTLE SPLIT SIGNED ON LOG PAGE. PERFORMED AMDS CARDS 2120-2367 SMOKE/ODOR TROUBLESHOOTING AND 7100-2619 PNEU DUCT BURN OUT. NO ORDERS NOTED.
DURING TAXI-OUT CREW REPORTED ODOR AND HAZE IN THE CABIN AND RETURNED TO THE DEPARTURE GATE. MAINTENANCE FOUND A HYDRAULIC LEAK AT THE LEFT ENGINE FILTER MODULE PRESSURE LINE FITTING. THE FITTING O-RING WAS REMOVED AND REPLACED AND A GOOD LEAK CHECK ACCOMPLISHED. COMPLETED MINI DUCT BURNOUT AND DEFERRED THE APU FOR CLEANING.

THE RED AFT CARGO BIN FIRE LIGHT ILLUMINATED BRIEFLY, EXTINGUISHED FOR ABOUT 30 SECONDS, AND THEN CAME BACK ON AND STAYED ON. MAINTENANCE INSPECTED THE AFT CARGO BIN AND FOUND NO EVIDENCE OF SMOKE OR FIRE. MAINTENANCE DID FIND LUGGAGE WEDGED AGAINST SMOKE DETECTOR NR 2. AS A PRECAUTION SMOKE DETECTOR NUMBER 2 AND THE OVERHEAD CARGO SMOKE CONTROL PANEL WERE BOTH REPLACED. THE AIRCRAFT WAS RETURNED TO SERVICE.

L1 DOOR SLIDE NOT CONNECTED TO GIRT BAR. SLIDE TO GIRT BAR. CHECKS OK. AIRCRAFT OK FOR SERVICE.
CREW REPORTED STRONG BURNING RUBBER ODOR IN CABIN AFTER TAKEOFF. SUSPECTED TIRE ISSUE AND DECLARED EMERGENCY FOR LANDING. MAINTENANCE FOUND NR 2 MAIN TIRE TREAD SEPARATED. REPLACED NUMBER 1 AND 2 MLG WHEEL/TIRE ASSEMBLIES. ACFT WAS FERRIED TO MSP FOR REPAIRS TO FLAP, BUTE DOOR, WING T/E PANEL, AND MLG SPRAY DEFLECTOR. ACFT HAS RETURNED TO SERVICE.
CREW REPORTED MINIMAL BRAKING AVAILABLE DURING LANDING WITH MANUAL APPLICATION. A POST FLIGHT INVESTIGATION REVEALED A BRAKE TEMP DIFFERENCE BETWEEN THE OTBD, HOT AND INBD, COOL. MAINTENANCE FOUND CONNECTORS ON NR 3 WHEEL TRANSDUCER AND NR 2 BRAKE SERVO BADLY CORRODED, BOTH CONNECTORS WERE REPLACED WITH SATISFACTORY CHECK OUTS.
ON TOUCHDOWN, CREW NOTED DEGRADED BRAKING ACTION WITH NORMAL BRAKE APPLICATION. FULL BRAKE PEDAL APPLICATION RESULTED IN PARTIAL BRAKING FORCE ACCORDING TO THE FLIGHT CREW. OBSERVED BRAKE TEMPS WERE NOT BALANCED BUT NO ASSOCIATED WARNINGS ON THE EOAP . MAINTENANCE CHECKED ANTI-SKID FUNCTION WITH NO FINDINGS BUT REPLACED THE ANTI-SKID CONTROL UNIT. FUNCTIONAL CHECKS WERE GOOD.

DURING LINE CHECK, MAINTENANCE FOUND THAT THE OVERHEAD EMERGENCY LIGHTING AT THE FORWARD EXIT AND FOR ROWS 1-14 OF THE CABIN WERE INOPERATIVE. MAINTENANCE REPLACED A FAILED FUSE AT THE POWER SUPPLY AND REPAIRED A DAMAGED WIRE, DUE TO BEING PINCHED IN THE WALL, AT DOOR 2R BY SPLICING.
DURING LINE CHECK, MAINTENANCE FOUND THAT THE EMERGENCY LIGHTING AT SEAT 18E WAS INOPERATIVE. MAINTENANCE FOUND WIRES PULLED OUT OF THE LIGHT SOCKET. REPAIRED AND THE SYSTEM RETURNED TO NORMAL OPERATION.
DURING SERVICE CHECK, MAINTENANCE FOUND THAT THE EMERGENCY PATHWAY LIGHTING IN THE FRONT SECTION OF THE CABIN, FORWARD DOORS TO ROW 11, HAD FAILED. MAINTENANCE REPAIRED A WIRE CONNECTOR FOR THE NOTED LIGHTED AREAS AND THE FOLLOW-UP, OPS CHECK WAS GOOD.
DURING SERVICE CHECK, MAINTENANCE FOUND THAT THE EMERGENCY LIGHTS IN THE FRONT SECTION OF THE CABIN, ROWS 1 - 12, HAD FAILED. MAINTENANCE REPLACED THE BATTERY PACKS AND OPS CHECKED GOOD.
DURING LINE CHECK, MAINTENANCE FOUND THAT THE EMERGENCY SEAT LIGHTS IN THE FIRST CLASS CABIN WERE INOPERATIVE. MAINTENANCE FOUND THAT A FUSE WAS LOOSE. THE FUSE WAS RE-SECURED AND THE LIGHTS RETURNED TO AN OPERATIONAL CONDITION.

DURING LINE CHECK, MAINTENANCE FOUND THAT THE EMERGENCY LIGHTING IN THE VENTRAL STAIRWAY INOPERATIVE. MAINTENANCE REPLACED THE BATTERY PACK AND THE LIGHTS RETURNED TO AN OPERATIONAL CONDITION.
PILOT DISCOVERED ONE TAILCONE RELEASE LATCH RECEPTACLE WAS POPPED AND ANOTHER WAS NOT FULLY LATCHED. THE RECEPTACLES WERE REATTACHED AND THE SYS CHECKED GOOD.
FLIGHT CREW DIVERTED THE ACFT AFTER THE COCKPIT AND CABIN EXPERIENCED SMOKE WHEN THE ENGINE ANTI-ICE VALVES WERE TURNED ON. CREW TURNED OFF ANTI-ICE SYS AND THE SMOKE DISSIPATED. MX CLEANED FLUID FROM THE WHEEL WELLS, LOWER FUSELAGE, AND APU INLET. PERFORMED OPN 2903 AND OPN 5088 AND NO FURTHER FAULTS WERE IDENTIFIED. ALSO COMPLIED WITH OPN 5090 AS AN ALT DUCT BURNOUT AND NO SMOKE OR ODOR WAS DETECTED. ENGINE ANTI-ICE OPS TEST OF BOTH ENGINES RESULTED IN NO FINDINGS.
DURING LINE INSPECTION, THE FWD OVERWING EMERGENCY EXIT LIGHTS WERE FOUND TO BE INOPERATIVE. TROUBLESHOOTING INDICATED THE BATTERIES WERE DISCHARGED AND NEEDED REPLACEMENT. BATTERIES WERE REPLACED AND TESTED IAW AMM 33-53. OPS NORMAL AFTER BATTERY REPLACEMENT NR 30 DAY HISTORY FOR THIS DISCREPANCY.
DURING LINE CHECK, MX FOUND THAT FWD OVERWING EMERGENCY EXIT SIGNS WERE INOPERATIVE. MX REPLACED THE BATTERY PACK AND THE LIGHTS RETURNED TO AN OPERATIONAL CONDITION.

REPAIRED ON ER/A 478044-14 ORIG: VERTICAL STABILIZER / FRONTSPAR / RT AND LT CHORD / ZFS=52 / FATIGUE CRACKS.

FOUND APU INLET SOAKED WITH DE-ICING FLUID AND OIL MIX. DURING TAKEOFF DUCTS GOT HOT CAUSING BURNING SMELL IN CABIN. INOP APU.
SMOKE AND NOXIOUS FUMES IN MID CABIN AREA AFTER T/O. FOUND CABIN AIR RECIRCULATION FAN SIEZED. REPLACED FAN, RAN FAN AND AC PACKS TO PURGE DUCTS.
ELECTRICAL SMELL AFTER TAKEOFF, CONTRACT MAINTENANCE INSPECTED APU INLET AND SURROUNDING AREAS. NO FLUIDS NOTED, RAN PACKS, NO SMELL NOTED.
SET UP TO REPLACE BOTH WATER SEPARATOR BAGS DUE TO SMOKE IN CABIN IN RSW - REPLACED SAME - RAN PACKS WITH ENGINE AIR - NO SMOKE NOTED.
