Probable cause

A COMPLETE FAILURE OF THE BRAKE SYSTEM DUE TO A CRACK IN THE BRAKE LINES. CONTRIBUTING TO THE ACCIDENT WAS THAT THE BRAKES WERE NOT ADEQUATELY MAINTAINED BY COMPANY MAINTENANCE PERSONNEL.
