THE OVERHEATING OF THE NUMBER 2 AIR CYCLE MACHINE WHICH RESULTED IN AN EMERGENCY EVACUATION VIA SLIDES; THE FAILURE OF THE FLIGHT ATTENDANTS TO SECURE THE SLIDE GIRT BARS.
