Probable cause

THE CAPTAIN'S INADEQUATE COORDINATION WITH THE GROUND AND FLIGHT CREW. FACTORS RELATED TO THE ACCIDENT WERE: THE PASSENGER'S PERCEPTION THAT THE APU EXHAUST WAS A FIRE, WHICH RESULTED IN ACTION THAT INITIATED AN EVACUATION.
