Probable cause
THE ASSIGNED MECHANIC'S FAILURE TO REINSTALL THE ATTACHMENT SCREWS AND THE QUALITY INSPECTOR'S INADEQUATE INSPECTION OF THE COMPLETED WORK. A FACTOR WAS THE COMPLACENCY OF THE MAINTENANCE PERSONNEL.
