THE PILOT'S FAILURE TO REMOVE ICE FROM THE AIRFRAME PRIOR TO TAKEOFF. FACTORS WERE FREEZING RAIN THE NIGHT BEFORE AND THE PILOTS' INCOMPLETE PREFLIGHT INSPECTION.
