THE PILOT'S PREMATURE LIFT-OFF DURING THE TAKEOFF, LEADING TO AN AERODYNAMIC STALL. A FACTOR CONTRIBUTING TO THE ACCIDENT WAS THE PILOT'S ATTEMPT TO PERFORM A FORMATION TAKEOFF.
