A total loss of engine power due to the failure of the engine’s Nos. 3 and 4 bearings and the power turbine pinion gear, and subsequent loss of main rotor rpm, which resulted in a loss of control and impact with the water.

A total loss of engine power due to the liberation of a second stage turbine blade near the blade root due to a high-cycle fatigue crack and subsequent overload. Although extensive testing and materials analysis was performed, the reason for crack initiation could not be determined.

AFTER 30 MINUTES IN CRUISE FLIGHT, TOT SUDDENLY WENT TO 840 CA, PRECAUTIONARY LANDING WAS MADE AND MAINTENANCE WAS CALLED. TOT SYSTEM WAS TESTED AND FOUND TO BE WORKING NORMAL. NO EXTERNAL DAMAGE WAS NOTED TO ENGINE. ENGINE WAS R & R AND SENT TO ENGINE SHOP FOR INSPECTION.
