The pilot’s failure to maintain directional control of the airplane, and the flight instructor’s inadequate oversight during a simulated engine out takeoff that resulted in a runway excursion and impact with a snowbank.

A loss of engine power due to fuel starvation as a result of the improper installation of the fuel nozzle. Also causal was the engine manufacturer's improper installation instructions for the nozzle pressure port plug on the fuel manifold valve. A factor associated with the accident was the corn crop which was contacted during the forced landing.

TALKED TO TECH REP AT MFG TOLD ME THAT PPG HAD PROBLEMS WITH THIS SERIES OF WINDSHIELD MFG AROUND 1999-2000. INSTALLED NEW WINDSHIELD, PN 101-384025-23. CRACKED WINDSHIELD WAS INSTALLED 1/19/2001.
