THE PILOT'S FAILURE TO REFUEL THE AIRPLANE PRIOR TO FUEL EXHAUSTION, AND HIS FAILURE TO MAINTAIN AIRSPEED RESULTING IN AN AERODYNAMIC STALL. HIS INADEQUATE TRAINING WAS A FACTOR IN THE ACCIDENT.
