Compressor stalls of both engines due to in-service compressor contamination. Factors in the compressor stall of the right engine were an atmospheric temperature inversion and a transient fuel flow condition produced when the constant thrust on takeoff (CTOT) system was disabled by the pilot's inadvertent movement of the right engine power lever during execution of the emergency procedure.

THE PILOT'S INABILITY TO RETARD THE POWER LEVERS DURING A REJECTED TAKEOFF DUE TO HIS FAILURE TO PERFORM THE EMERGENCY PROCEDURES IN ACCORDANCE WITH THE AIRCRAFT OPERATING MANUAL. A FACTOR WAS THE ILLUMINATION OF THE CONFIGURATION LIGHT AS RESULT OF INSUFFICIENT REBOUND ON THE RIGHT CONDITION LEVER WHILE PLACED IN THE FULL FORWARD POSITION.

ENGINE ICE/SLUSH INGESTION THAT RESULTED IN POWER INTERRUPTIONS AND FAILURE OF THE AIR DATA COMPUTER COUPLED WITH MOISTURE INGESTION OF THE STATIC SYSTEM. FACTORS WERE THE FAILURE OF THE MANUFACTURER TO CORRECT A PREVIOUSLY IDENTIFIED DESIGN DEFICIENCY THAT PERMITTED THE ACCUMULATION OF ENGINE INLET ICE/SLUSH, WHICH COULD NOT BE OBSERVED BY THE FLIGHT CREW, NOR PREVENTED BY ON-BOARD ENGINE ANTI-ICE EQUIPMENT, THE LACK OF A PROACTIVE STANCE BY THE FAA WHEN INVOLVED WITH AIRCRAFT CERTIFICATION UNDER BI-LATERAL AGREEMENTS, AND THE INADEQUACY OF THE FEDERAL AVIATION REGULATIONS REGARDING ENGINE INLET ICE PROTECTION.

THE INSTRUCTOR/PILOT-IN-COMMAND'S FAILURE TO ADEQUATELY SUPERVISE THE FLIGHT AND THE SECOND PILOT'S IMPROPER USE OF THE CHECKLIST AND FAILURE TO MAINTAIN CREW COORDINATION TO ENSURE THAT THE LANDING GEAR WAS EXTENDED. CONTRIBUTING TO THE ACCIDENT WAS THE SILENCING OF THE LANDING GEAR WARNING HORN BY POSITIONING GO THE ENGINE POWER LEVERS.
