Part: LINE — FAILED (HYD SYSTEM)

THE HYDRAULIC PRESSURE LINE FROM THE PUMP HAD FAILED. THE CLAMP THAT SECURES THE HOSE TO THE PYLON DECK HAD CHAFED INTO THE STEEL BRAIDING OF THE LINE, CAUSING A WEAK POINT IN THE LINE, AND THE WALL OF THE HOSE TO RUPTURE THROUGH THE BRAIDING. THUS CAUSING A TOTAL HYDRAULIC FAILURE. THE CAUSE OF THE HOSE FAILURE IS THE ALIGNMENT OF THE CLAMP ON THE HOSE.

COMPLETED 60 MONTH MAST INSPECTION, FOUND CORROSION ON MAST POLE IN AREA ON RETAINING NUT THREADS, AND THE RETAINING NUT THAT WAS BEYOND ALLOWABLE LIMITS IAW PRODUCT SUPPORT EVALUATION. REPLACED MAST POLE AND RETAINING NUT. THERE WAS NO TORQUE ON THE RETAINING NUT WHEN REMOVED FOR THE INSPECTION AND THE PRO-SEAL WAS NOT SEALING AGAINST THE MAST POLE, THUS CAUSING MOISTURE AND CORROSION ON THE POLE AND RETAINING NUT.
