Part: FUEL CONTROL — MALFUNCTIONED (RT ENGINE) · airframe 9,506 hrs · 8,233 cycles
RT ENGINE FUEL CONTROL MALFUNCTION. NO POWER LEVER RESPONSE ON THE RT ENGINE. COULD NOT REDUCE BELOW 900 DEG C. DIVERTED. ON APPROACH , SHUTDOWN ENGINE AND LANDED WITH NO FURTHER PROBLEMS.

DURING GEAR EXTENSION FOR LANDING, A LOUD BANG WAS NOTICED BY THE PIVOT AND UNSAFE LIGHT STAYED ON. AIRCRAFT WAS ROUTED TO MAINTENANCE AFTER LANDING. MAINTENANCE FOUND ROD END ART -7E ON SHAFT HAD COME APART ANDALLOWED DOORS TO BECOME DISCONNECTED FROM ACTUATOR. (SW15200706339) (K)

PILOT REPORTED DURING CRUISE LEVEL FLIGHT FEELING A SUDDEN VIBRATION AND HEARING A SCREAMING NOISE FROM THE LT ENGINE. PILOT THEN NOTICED TORQUE INDICATION ON LT ENGINE INDICATING 120 PERCENT WITH NO YAW INDICATED OR NOTICED BY PILOT. WHILE RETARDING LT POWER LEVER TO REDUCE TORQUE THE LT ENGINE SHUTDOWN UNCOMMANDED. ENGINE NTS SYSTEM FUNCTIONED NORMALLY AND AN UNEVENTFUL SINGLE ENGINE LANDING WAS MADE. UPON ENGINE ASSEMBLY A 2 INCH PIECE OF THE BULL GEAR OUTER RIM WAS LAYING IN THE BOTTOM OF THE ACCESSORY HOUSING. INSPECTION OF THE BULL GEAR REVEALED THE MISSING SECTION AND AN APPROX 4 INCH CRACK IN THE HUB AREA WHICH CAUSED LOSS OF DRIVE TO THE PROPELLER SHAFT.

ENGINE, SN P20186, EXPERIENCED AN UN-COMMANDED INFLIGHT SHUTDOWN, ON SITE INSPECTION REVEALED THAT THE FUEL PUMP DRIVE TRAIN HAD BECOME UNCOUPLED. TEARDOWN INSPECTION REVEALED FAILURE OF SPUR GEARSHAFT PN 8968042, COMPONENT OF TORQUE SENSOR PN 31017262, SN P2648 WITH 3369.8 TSO. THIS ARICRAFT IS IN COMPLIANCE OF ALL APPLICABLE AD. (K)
