FACTORS RELATED TO THE ACCIDENT WERE: THE ADVERSE WEATHER CONDITIONS (SNOW FALLING & HIGH DENSITY ALTITUDE), THE LACK OF SNOW COVERS TO PROTECT THE ENGINE INTAKES WHILE PARKED AND AWAITING FOR THE WEATHER CONDITIONS TO IMPROVE, IMPROPER PLANNING/DECISION BY THE PILOT AND THE COMPANY/OPERATOR MANAGEMENT, INADEQUATE INFORMATION IN THE FLIGHT MANUAL, AND THE LACK OF SUITABLE TERRAIN FOR AN EMERGENCY LANDING.

THE FAILURE OF THE PILOT TO MAINTAIN ROTOR RPM DURING THE PREMATURE LIFT-OFF WHICH RESULTED IN A FORCED LANDING. CONTRIBUTING TO THE ACCIDENT WAS THE HIGH DENSITY ALTITUDE CONDITIONS, AND UNSUITABLE TERRAIN.

THE FREEWHEELING UNIT WAS FAILING CAUSING THE MAIN GEARBOX CHIP LIGHT TO INTERMITTENTLY ILLUMINATE. WHEN THE CHIP DETECTORS WERE INSPECTED THERE WAS NO METAL NOTED. NOTHING APPEARED IN THE OIL FILTER OR SCREEN DURING THE SERVICEABILITY CHECK.
