DURING CRUISE FLIGHT, A LOUD NOISE WAS HEARD COMING FROM THE ENGINE COMPARTMENT. FOLLOWED BY AN AIRFRAME VIBRATION, WHICH WAS FOLLOWED BY AN ENGINE SHUT DOWN. AFTER LANDING A LARGE HOLE WAS FOUND IN THE ENGINE CRANKCASE ABOVE THE NR 5 CYLINDER. DURING THE TEARDOWN INSPECTION, IT WAS NOTED THAT THE PISTON COOLING NOZZLE ASSEMBLIES HAD NOT BEEN INSTALLED. IT APPEARS THAT SOMETIME DURING THE ASSY OF THE ENGINE, A STD-1339, PLUG WAS INSTALLED INSTEAD OF A COOLING NOZZLE. THE PLUG EFFECTIVELY BLOCKS OFF THE FLOW OF OIL TO THE BASE OF THE CYLINDERS PREVENTING THEM FROM GETTING ANY ADDITIONAL COOLING AND LUBRICATION.

THE LOOP ASSEMBLY IN THE UPPER SILL PLATE OF THE FUSELAGE AFT CABIN DOOR BECAME LOOSE, ALLOWING THE UPPER LATCH AT THE TIP OF THE DOOR TO RELEASE DURING CRUISE FLIGHT. THE TOP PORTION OF THE DOOR INCLUDING THE WINDOW, SUBSEQUENTLY BROKE OFF, DENTING THE LT INBD LEADING EDGE STABILATOR SKIN BETWEEN THE 2 MOST INBD RIBS. NO FURTHER DAMAGE WAS NOTED, AND FLIGHT CHARACTERISTICS REMAINED UNAFFECTED. PRIOR TO FLIGHT THE DOOR APPEARED TO BE SECURELY LATCHED.

CRACKS WERE FOUND AT THE TOP OF THE RUDDER SPAR HINGE DOUBLER. CRACKING WAS ON BOTH SIDES OF THE DOUBLER IN THE BEND RADIUS`S. ACCESS FOR INSPECTION NECCESSITATES REMOVING THE RUDDERS TIP/COUNTERWEIGHT. RECURRENCE MAY BE PREVENTED BY FABRICATING AND INSTALLING AN ADDITIONAL SIMILAR DOUBLER ON THE AFT SIDE OF THE SPAR OPPOSITE THE EXISTING DOUBLER. HOWEVER, THAT MAY EFFECT FLEXIBILITY AND CAUSE THE RUDDER SPAR TO RECEIVE EXTRA STRESS. (AL03200519295) (K)
