The mechanic’s failure to properly torque a fuel control unit B-nut, which resulted in a partial loss of engine power, an attempted autorotation, and an impact with terrain.

The pilot's failure to maintain adequate separation between a long line being used to hoist linemen from a shield wire during an external load flight, which resulted in the long line being severed.

The pilot's failure to clear the helicopter and its external load from surrounding structures and his failure to execute a timely release of the external load, which resulted in an inadvertent entanglement and collision with wires and terrain. Contributing to the lineman's fatal injury was his position outside of the helicopter during the accident.

Part: STRAP — CRACKED (MAIN ROTOR)
MAIN ROTOR HUB STRAP PACK ASSY WAS FOUND WITH ONE CRACKED LEG AT 226.2 HOURS TSO, SECOND CRACK FOUND AT 542.1 HOURS. BOTH CRACKS WERE IN THE LAG (TRAILING EDGE) LEG OF THE SAME PACK. THIRD AND FAILING CRACK WAS FOUND AT 578.8 HOURS TSO. REPLACED WITH ZERO HRS TSO ITEM. THIS AIRCRAFT HAS BEEN USED FOR THE SAME ACTIVITIES (POWERLINE CONSTRUCTION AND MAINTENANCE) FOR THE PRECEDING 6,000 HRS AND THIS IS THE FIRST STRAP PACK FAILURE (ON LAST 3 HUB ASSY'S). ALSO, OPERATE 4 OTHER HUGHES 369 SERIES ACFT AND NO STRAP PACK FAILURES ON ANY OF THOSE EITHER. SB DN-154 COVERS THE INSPECTION OF THIS COMPONENT. STRAP S/N'S F707 THROUGH F7111.

AIR CONDITIONER BRACKET CRACKED. THIS IS THE UPDATED BRACKET MADE OUT OF STEEL. WHEN BRACKET WAS MADE OUT OF ALUMINUM, HAD ALOT OF FAILURES ALL IN THE SAME AREA.

THIS MODEL OF TURN AND SLIP INDICATOR HAS A HIGH FAILURE RATE. (BOTH THE PILOT'S AND COPILOT'S USE THE SAME INDICATOR.) SUBMITTER STATES CHANGED 6 UNITS SINCE THE FIRST OF THE YEAR. THE HIGH TIME UNIT HAS 45 HOURS ON IT SINCE OVERHAUL, MOST HAD LESS.

PIVOT BRACKET HOLDS AIR CONDITIONER COMPRESSOR TO ENGINE ACC SECTION. PART IS MADE OUT OF ALUMINUM. EARS KEEP BREAKING OFF. RECOMMEND REDESIGN. SUBMITTER STATED THIS IS THE FOURTH BRACKET TO BREAK, ALL IN EXACT SAME LOCATION. BRACKET 1,069 HRS TT, FIRST ONE. SECOND, 414 HRS TT. THIRD, 519 HRS TT. FOURTH, 1497.9 HRS TT.
Part: SWITCH ACTUATOR — BROKEN (CABIN DOOR WARN)
THE SWITCH ACTUATOR ON THE CABIN DOOR LIGHT WAS BENT BACK AND BROKEN OFF. THE CONDTION WAS DUPLICATED BY PUSHING DOOR BUTTON AND MOVING HANDLE FURTHER INTO THE CLOSED POSITION. FOLLOWER FALLS BEHIND LOCK TAB. HANDLE IS THEN ROTATED OPEN FOLLOWER IS BENT. THUS, A DOOR UNSAFE LIGHT. BRACKETS IN THIS AREA ARE LIGHT WEIGHT AND MOVE AROUND.
Part: SEAT — LOCK MALFUNCTION (CABIN)
SEAT LOCKING MECHANISM IS INADEQUATE. SEAT CAN BE OVERPOWERED WITH THE SLIGHTEST BODY MOVEMENT. RE-ADJUSTED PER FACTORY SPECS. WITH NO IMPROVEMENT. SUBMITTER STATED OTHER OPERATORS ARE HAVING SAME PROBLEMS.
