ON APRIL 12, 2025, WHILE AIRCRAFT N171AM WAS TAXIING FROM THE RAMP AT FORT LAUDERDALE EXECUTIVE AIRPORT (FXE), THE FLIGHT CREW OBSERVED AN EICAS MESSAGE INDICATING âSTAB CH 2 INOP.â THE CREW ATTEMPTED TO RESET THE SYSTEM; HOWEVER, THE RESET WAS UNSUCCESSFUL. THE AIRCRAFT SUBSEQUENTLY RETURNED TO THE RAMP FOR MAINTENANCE EVALUATION AND CORRECTIVE ACTION.
CREW REPORTED AFTER DEPARTURE WHEN PILOT TRIED TO TRIM STABILIZER BOTH CHANNELS DISCONNECTED. AFTER ABNORMAL CHECKLIST WAS PERFORMED CREW WAS ABLE TO CONNECT CHANNEL 2. CREW RETURNED TO FIELD AND LANDED WITHOUT INCIDENT. MAINTENANCE TROUBLESHOOT TO FAULT HSTCU BOX.
DURING A NORMAL CLIMB-OUT AFTER A TAKEOFF ON RUNWAY 25 AT KSBA, "POSITIVE RATE" WAS CALLED BY THE PM (SIC), AND THE PF (PIC) RESPONDED WITH "GEAR UP, YAW DAMPER". UPON GEAR RETRACTION, THE CREW NOTICED THAT THE NOSE RED "UNSAFE" LIGHT REMAINED ILLUMINATED. THIS WAS ALSO ACCOMPANIED BY A SLIGHT INCREASE IN AIRFLOW NOISE COMPARED TO NORMAL GEAR RETRACTION. THE CREW LEVELED OFF AT 2,000 FEET AND BEGAN TO TROUBLE SHOOT. THE GEAR WAS CYCLED TWICE, AND BOTH TIMES THE SAME INDICATION PRESENTED. AFTER THIS, THE CREW DID A FLY-BY WITH TOWER AND THE TOWER COMMENTED THAT THE NOSE GEAR DOORS WERE OPEN AND THE NOSE GEAR APPEARED ONLY PARTIALLY RETRACTED. WHEN THE GEAR WAS LOWERED, THE CREW HAD "3 GREEN, NO RED" INDICATING ALL 3 GEAR WERE DOWN AND LOCKED. AFTER DECIDING THAT THE BEST COURSE OF ACTION WAS TO RETURN TO KSBA AND LAND, THE CREW FLEW AROUND FOR APPROXIMATELY 1.5 HOURS (ON VECTORS WITH SBA APPROACH CONTROL) IN ORDER TO BURN ENOUGH GAS TO COME DOWN BELOW MAXIMUM LANDING WEIGHT. ONCE THE CREW WAS WITHIN MAX LANDING WEIGHT, ONE MORE FLY-BY WAS DONE WITH THE GEAR EXTENDED. THE TOWER COMMENTED THAT ALL 3 GEAR APPEARED TO BE DOWN, LOCKED, AND STRAIGHT. THE CREW BROUGHT THE AIRPLANE AROUND THE PATTERN AND CAME TO A FULL STOP LANDING. EMERGENCY EQUIPMENT/PERSONNEL WERE STANDING BY JUST IN CASE. LANDING WAS UNEVENTFUL. NOSE GEAR HELD UP AND EVEN ALLOWED FOR NOSE STEERING. THE CREW TAXIED THE AIRCRAFT BACK TO PARKING AND SHUT DOWN.
DURING A NORMAL TAKEOFF ROLL ON 19L AT KLAS ONE OF THE LEFT MAIN TIRES BLEW DUE TO A SUSPECTED ENCOUNTER WITH FOD AT APPROXIMATELY 105 KNOTS WITH A V1 TAKEOFF DECISION SPEED OF 136; THIS WAS ASSOCIATED WITH AN AUDIBLE "BANG". SHORTLY THEREAFTER THE AIRCRAFT BEGAN TO LOSE DIRECTIONAL CONTROL AND THE CAPTAIN ELECTED TO ABORT THE TAKEOFF. THIS SERIES OF EVENTS CAUSED THE REMAINING LEFT TIRE TO BLOW AND CAUSED THE LEFT WHEEL ASSEMBLIES AND BRAKES TO DRAG ON THE RUNWAY DURING THE ABORT DECELERATION AND STOP. THE AIRCRAFT REMAINED ON THE RUNWAY THE ENTIRE TIME. AFTER THE AIRCRAFT CAME TO A STOP ON RUNWAY 19L THE CREW PERFORMED A PRECAUTIONARY EVACUATION AND SHUTDOWN OF THE AIRCRAFT.
ON DEPARTING KCXY "CABIN ALT" LIGHT ILLUMINATED PASSING THROUGH 10,000 FEET. CAPTAIN ELECTED TO LAND AT KMDT INSTEAD OF THE FILED DESTINATION (FXE).
INDICATION OF NO PITOT HEAT ON ANNUNCIATOR PANEL.
BATTERY, CONTINUALLY WEAK ON STARTS.
CLAMP WELDED TO TUBE CRACKED, LEAVING TUBE UNSUPPORTED.
CLAMP WELDED TO TUBE, CRACKED, LEAVING TUBE UNSUPPORTED.
FIBERGLASS GASKET INSTALLED ON BOTH SIDES OF RING GEAR CRACKED AND PUSHED OUT CAUSING AN OIL LEAK.
