MODIFIED AIRCRAFT WITH AN STC FOR GOGO LX5 TO CONNECT THE AIRCRAFT TO A 5G NETWORK FOR IN FLIGHT ENTERTAINMENT. ALL WORK COMPLETED PER WIRING DRAWINGS SUPPLIED AND WERE CURRENT AT THE TIME. IT WAS FOUND LATER THAT THE ON / OFF SWITCH WIRING (DRAWING CA25174) SHOULD BE ON ITS OWN BREAKER WHILE ALSO BEING A SMALLER BREAKER TO PROTECT THE AIRCRAFT AND WIRING. IN THE PREVIOUS WIRING DRAWING FOR THIS SWITCH, THERE WAS A SAFETY RISK THAT THE BREAKER WOULD NOT OPEN AND THE CIRCUIT COULD OVERHEAT. TEXTRON AVIATION ENGINEERING TEAM UPDATED DRAWING TO REFLECT THE CHANGE AND CORRECT THE ENGINEERING MISTAKE, AND AIRCRAFT HAS BEEN CORRECTED IN THE FIELD. ORIGINAL DRAWING ENGINEERED BY LIBERTY PARTNERS ON BEHALF OF TEXTRON AVIATION. THOUGH THERE WAS NO PROBLEMS FOUND WITH THE AIRCRAFT ONCE DISPATCH TO CORRECT THE ISSUE, WE FEEL AS IT IS OUR OBLIGATION TO REPORT A SERVICE DIFFICULTY REPORT. WHILE COMING FROM SDF LOUISVILLE KENTUCKY AT AN ALTITUDE OF 29000 FT ON DESCENT THE COPILOT'S WINDSHIELD SHATTERED. PILOT TOOK OVER AND ABOUT 2 MINUTES LATER THE PILOT'S WINDSHIELD ALSO SHATTERED. THEY CONTINUED THE FLIGHT INTO PIT WITH BOTH WINDSHIELDS SHATTERED. WE TALKED TO LEE AEROSPACE INDUSTRIES AND THEY SAID THEY HAVE BEEN HAVING ISSUES WITH THE ANTISTATIC COATING ON THE WINDSHIELDS. THE PILOTS REPORTED SEEING ST ELMO'S FIRE AFTER THEY SHATTERED.

WHILE LANDING THE CREW EXTENDED LANDING GEAR INTO SBN. THEY HEARD A POP IN THE NOSE. THEY HAD ALL GREEN INDICATION AND LANDED. CREW NOTICED THAT IT WAS HARD TO STEER. UPON INSPECTION FOUND ONE OF THE SPADE DOOR HINGES CRACKED OFF.

LEFT HAND TIRE TREAD DEPARTED THE TIRE AFTER TAKEOFF. THE CREW GOT A ANTISKID FAILURE CAS MESSAGE DISPLAYED SHORTLY AFTER TAKEOFF. THE CREW DISCOVERED THAT THE FLAPS WERE SLIGHTLY AFT OF THE FULLY STOWED POSITION. THE HYDRAULIC AMBER PRESSURE CAS MESSAGE STAYED ON AND THE CREW WENT THROUGH THE CHECKLIST AND ENTAILED PULLING BREAKER FOR HYDRAULIC CONTROL BREAKER SO THE PUMP DOESN'T RUN CONTINUOUSLY. AFTER A NORMAL APPROACH AND LANDING GOING INTO KCMH, IT WAS DISCOVERED THAT THE TIRE TREAD HAD LEFT THE TIRE AND DAMAGED THE FLAP DOING SO, ALONG WITH THE ANTISKID WIRE.

ENROUTE, NR 2 ENGINE EXPERIENCED AN UN-COMMANDED SHUTDOWN. MAINTENANCE REMOVED AND REPLACED THE FUEL DELIVERY UNIT (FDU) AS A PRECAUTION AS NO ROOT CAUSE OF THE PROBLEM WAS IDENTIFIED.
