THE AIRCRAFT SUFFERED AN INFLIGHT ENGINE OVERSPEED AND SUBSEQUENT ENGINE FAILURE. THE PILOT PERFORMED A SUCCESSFUL AUTOROTATION TO THE GROUND WITH NO DAMAGE TO THE AIRFRAME AND NO INJURIES TO THE OCCUPANTS. AFTER RECOVERY OF THE ACFT AND REMOVAL. INSPECTION OF THE ENGINE REVEALED THAT THE FCU SUFFERED A CATASTROPHIC DRIVE BEARING FAILURE WHICH WAS THE ROOT CAUSE OF THE ENGINE FAILURE.
THE HELICOPTER WAS CONDUCTING A SNOW SURVEY, AT APPROXIMATELY 11,500FT, AN ENGINE CHIP LIGHT ILLUMINATED ON THE CAUTION PANEL OF THE ACFT RIGHT AFTER TAKEOFF. THE PILOT IMMEDIATELY LANDED AND PROCEED TO INSPECT THE MAGNETIC ENGINE CHIP PLUGS WHICH WERE FOUND TO BE FULL OF METAL. AN ENGINE OIL ANALYSIS WAS PERFORMED 306 HRS PRIOR TO THIS INCIDENT AND FOUND NO ABNORMAL LEVELS OF METAL IN THE ENGINE OIL. THE M01, M02, M04 AND M05 ENGINE MODULES WERE FOR INSPECTION AND EVALUATION DUE TO METAL CONTAMINATION. FOUND THE CAUSE OF METAL CONTAMINATION TO BE THE REAR BEARING PN 9609000166 IN THE M04 MODULE. THE MANUFACTURER HAS NOT DISCLOSED THE REASON WHY THE BEARING FAILED OR PROVIDED ANY RECOMMENDATION TO PREVENT RECURRENCE. ATTACHED ARE PICTURES OF THE FAILED BEARING FROM THE TECHNICAL MAINTENANCE REPORT PROVIDED BY MANUFACTURER.
APPROXIMATELY 3PM LOCAL TIME, CONDUCTING A SNOW SURVEY AT APPROXIMATELY 11500 FT. AN ENGINE CHIP LIGHT ILLUMINATED ON THE CAUTION PANEL OF THE ACFT RIGHT AFTER TAKEOFF. THE PILOT IMMEDIATELY LANDED AND PROCEEDED TO INSPECT THE MAGNETIC ENGINE CHIP PLUGS WHICH WERE FOUND TO BE FULL OF METAL. AN ENGINE ANALYSIS WAS PERFORMED 3.6 HOURS PRIOR TO THIS INCIDENT AND FOUND NO ABNORMAL LEVELS OF METAL IN THE ENGINE OIL. THE M01, M02, M04 AND M05 ENGINE MODULES WERE SENT FOR INSPECTION AND EVALUATION DUE TO METAL CONTAMINATION. THE CAUSE OF METAL CONTAMINATION TO BE THE REAR BEARING IN THE M04 MODULE. MANUFACTURER HAS NOT DISCLOSED THE REASON WHY THE BEARING FAILED OR PROVIDE ANY RECOMMENDATIONS TO PREVENT A RECURRENCE.
DEFECTIVE HOSE, PILOT NOTICED LOSS OF HYD FLUID ON POST FLIGHT.
DEFECTIVE HYDRAULIC HOSE, INSTALLED NEW TYPE HOSES AND AFTER 24.0 HRS IT STARTED LEAKING.
DURING A ROUTINE 600 HR BORESCOPE INSP OF THE GAS GENERATOR TURBINE BLADES, 2 BLADES WERE FOUND TO HAVE MOVED AFT IN THEIR (FIR-TREE) TYPE MOUNTS IN THE TURBINE DISK. 1 BLADE WAS FOUND TO HAVE MOVED .1250-.2500 INCH, AND THE OTHER LESS THAN .1250 INCH. THE BLADES HAD TO MOVE PAST A RETAINING CLIP TO ACHIEVE THE AMOUNT OF MOVEMENT OBSERVED. THIS TYPE OF DEFECT IS NOT LISTED AS A POSSIBLE DEFECT IN THE ENGINE MM AND SMALL AMOUNTS OF INSP REGIME. DUE TO THE POTENTIAL EFFECTS OF THE RECOMMEND A BORESCOPE INSP OF THE BLADE ALIGNMENT BEFORE FURTHER FLIGHT AND AT AN INTERVAL OF 300 HOURS ENGINE IS BEING RETURNED TO MFG. (K)
