RIGHT HAND NOSE STEERING ACTUATOR IS LEAKING AT THE SHAFT SEAL.
DURING LANDING, NR 2 MAIN TIRE TREAD SEPARATED FROM THE TIRE, NO DAMAGE TO THE ACFT WAS REPORTED.
CREW NOTICED AN ODOR IN THE COCKPIT CLIMBING OUT, ENROUTE AND STILL PERSISTENT. THE CREW ELECTED TO MAKE A PRECAUTIONARY LANDING. MAINTENANCE INSPECTED THE ACFT AND ENGINE. THE ODOR COULD BE ISOLATED BY SHUTTING THE LT ENGINE BLEED. THE ACFT WAS FERRIED WHERE A BORESCOPE WAS PERFORMED, AND NO CONCLUSIVE FINDS. ENGINE MFG RECOMMENDS THE LT ENGINE CARBON SEAL ON THE LOANER ENGINE TO BE REPLACED AS A PRECAUTION.
LOUD SQUEALING DURING TAKEOFF AND CABIN PRESSURIZATION INDICATED 1500 TO 2000 FPM DOWN AND ABORTED TAKEOFF. TROUBLESHOOTING AND FURTHER ENGINE AND PRESSURIZATION RUNS COULD NOT DUPLICATE THE PROBLEM. BELIEVE TO HAVE BEEN A STICKY OUTFLOW VALVE DUE TO NOT FLYING IN FOR A LONG TIME.
DURING THE 10 YEAR, 8C INSPECTION, HEAD OF A LOCK FASTENER WAS FOUND MISSING ON A VERTICAL STIFFENER ON THE AFT SIDE OF THE REAR PRESSURE BULKHEAD. AFTER FURTHER INVESTIGATION A INCH CRACK WAS FOUND A THE TOP OF THAT VERTICAL STIFFENER AS WELL AS SEVEN LOCK FASTENERS MISSING FROM THE VERTICAL STIFFENER TO THE MAIN HORIZONTAL BEAM. VERTICAL STIFFENER REPLACED AND HIDDEN DAMAGE INSPECTION BEING PERFORMED AS WELL AS AN EDDY CURRENT INSPECTION OF THE AREA. THESE DEFECTS WERE FOUND WHILE PERFORMING THE VISUAL INSPECTION OF THE REAR PRESSURE BULKHEAD TASK 53-40-00-220-809 IN THE AMM.
THE PASSENGER DOOR INNER LOCKING HANDLE IS CRACKED AT THE TOP OF THE HANDLE, FROM THE KNOB EXTENDING DOWN THE HANDLE, ALL THE WAY THROUGH. SUSPECT CAUSE IS DUE TO FLEXING OF HANDLE AT KNOB AREA WHERE THE MATERIAL IS TRIMMED OUT. SUGGEST OEM REVISE DESIGN OF HANDLE TO IMPROVE STRENGTH.
CREW REPORTS NOSE STEERING FAIL LIGHT COMES ON AND STEERING IS UNRESPONSIVE WHEN TAXI MODE IS SELECTED. THIS IS THE 4TH ACTUATOR USED ON THIS ACFT IN 506.8 HOURS. ALL OTHER HAVE FAILED IN A SIMILAR MODE. THE PROBLEM SEEMS TO BE IN THE ELECTRICAL FOLLOW UP MECHANISM IN THE ACTUATOR. THIS STEERING SYS IS THE SUBJECT OF AD2007-18-07 ISSUED TO ADDRESS A SIMILAR SYMPTOM. AD 2007-18-07 HAS BEEN COMPLIED WITH ON ALL 4 ACTUATORS INSTALLED USED ON THIS ACFT YET FAILURE CONTINUE TO OCCUR. INS SOME FAILURE MODES THIS STEERING SYSTEMCAN MAKE UN-COMMANDED TURNS. (K)
