AIRCRAFT WAS IN CRUISE FLIGHT, THE AIR CONDITIONING WAS TURNED ON, A BURNING SMELL WAS SOON NOTICED, THE A/C WAS THEN TURNED OFF, THE SMELL IMMEDIATELY DISSIPATED. THE AIRCRAFT RETURNED TO BASE. TECHNICIAN INSPECTED THE A/C SYSTEM TO INCLUDE THE EVAP BLOWER, IT WAS NOTICED THE EVAP MOTOR DID NOT OPERATE IN THE LOW MODE. EVAP MOTOR WAS REMOVED AND INSPECTED. THERE ARE TWO RESISTORS MOUNTED INSIDE THE MOTOR CAGE ASSY, ONE FOR EACH HIGH/LOW SETTING. THERE APPEARED TO BE NO VISUAL INDICATIONS OF ARCING OR OTHERWISE, THERE WAS A SLIGHT BURNT SMELL IN THAT LOCATION. THE EVAP BLOWER ASSY WAS REPLACED WITH A NEW UNIT, ALL OPERATIONAL TESTS WERE NORMAL WITH NO DEFECTS NOTED. AIRCRAFT WAS RETURNED TO SERVICE.
AFTER AIRCRAFT LANDED AND DURING THROTTLE REDUCTION TO IDLE, THE NR 1 ENGINE FLAME DETECTOR FLICKERED ON AND OFF TWICE, APPROXIMATELY 1 SECOND EACH TIME. ACFT WAS SHUTDOWN, AND MAINTENANCE VISUALLY INSPECTED THE ENGINE AREA, NO EVIDENCE OF A FIRE WAS NOTED. A COCKPIT TEST OF THE FIRE DETECTION SYS WAS COMPLETED WITH NO DEFECTS NOTED. THE FLAME DETECTOR WAS REMOVED, VISUALLY INSPECTED, THE CONNECTOR PLUG AND DETECTOR WERE CLEANED. THE DETECTOR WAS SWAPPED WITH THE NR 2 SIDE TO FURTHER HELP IN TROUBLESHOOTING. A COMPLETE FIRE DETECTION OPS TEST USING THE SIMULATED FIRE TEST BOX WAS CONDUCTED WITH NO DEFECTS NOTED. THE ACFT WAS GROUND RUN AND FLOWN TWICE ALSO WITH NO DEFECTS NOTED. ACFT WAS RETURNED TO SERVICE.
SHORTLY AFTER LIFT OFF, HATCH ASSY SEPARATED FROM ACFT. RETURNED TO AIRPORT AND LANDED WITHOUT INCIDENT. FOUND LATCH PARTS; PIVOT, SPRING, AND ROD WERE ALL DISCONNECTED FROM CLEVIS AND LOCK NUT MISSING. FINDINGS INDICATE THE ROD VIBRATED LOOSE AND THE ASSY CAME APART.
DURING ROUTINE INSPECTION, THE UPPER LEFT STATION 300 TAIBOOM FITTING WAS FOUND TO BE CRACKED. THE CRACK APPEARED TO START AT THE BOLT HOLE AND EXTEND TO LOWER EDGE. THE FITTING WAS REPLACED IAW SIKORSKY STRUCTURAL REPAIR MANUAL AND THE AIRCRAFT WAS RETURNED TO SERVICE.
