The pilot’s inadequate visual lookout while departing from a multiple-use waterway, which resulted in a collision with a drifting boat. Contributing to the accident were the lack of visual conspicuity of the boat and the limited amount of time the boat operator had to maneuver the boat once he heard and saw the oncoming airplane.

Part: CONTROL HANDLE — MALFUNCTIONED (MLG) · airframe 12,648 hrs
LANDING GEAR LOWERED AND PILOT REPORTED INDICATOR LIGHTS WERE ALL 3 GREEN, THE LANDING GEAR LEVER DID NOT RETURN TO NEUTRAL. PILOT ATTEMPTED TO CYCLE GEAR UP, BUT NO RESPONSE. GEAR LIGHTS CONTINUED TO INDICATE GREEN (LOWERED AND LOCKED). PILOT FLEW BY TOWER TO VERIFY GEAR DOWN AND THEN LANDED WITHOUT INCIDENT.

DEPARTING SLC EXPERIENCED A RADIO PROBLEM, BOTH COM'S BECAME INTERMITTENT. CIRCLED THE AIRPORT MAINTAINING VFR, AND REESTABLISHED CONTACT WITH ATC FOR THE REMAINDER OF TIME. ELECTED TO LAND AT OGD. MX SUPERVISOR WAS NOTIFIED, AND LOCAL MX T/S THE PROBLEM. THE PILOTS HAND MIC JACK HAD BEEN GROUNDED THUS ACTING TO KEY THE MIC CONTINUOUSLY. THE JACK WAS SECURED, AND THE CO-PILOTS MICROPHONE WAS REPLACED. NUMEROUS RADIO CHECKS WERE MADE WITHOUT DIFFICULTY. THE AIRCRAFT WAS THEN RETURNED TO SERVICE. (M)

AKN - EN ROUTE AIRCRAFT EXPERIENCED ENGINE ROUGHNESS. PILOT LANDED AIRCRAFT AT REINDEER BAY, NAKNEK LAKE. PROBLEM WAS DETERMINED TO BE EXHAUST SIDE ROCKER CHAMBER CRACKED THROUGH ROCKER AXLE PIN BORES. SUSPECT CAUSES: COULD BE INCORRECT ROCKER AXLE TORQUE OR VALVE STICKING.
Part: MAGNETO — CONDENSER FAILED (ENGINE)
ENGINE DEVELOPED ROUGHNESS, DETERMINED TO BE MAGNETO FAILURE. PILOT LANDED AND TAXIED ON BEACH WHERE IT WAS DETERMINED THE EXHAUST ROCKER CHAMBER NR 1 CYLINDER HAD CRACKED, PRESUMABLY FROM ENGINE ROUGHNESS. BOTH COMPONENTS WERE REPLACED.
Part: CYLINDER — FAILED (NR 1)
ON COMPANY FLIGHT 8-27-96, AIRCRAFT WAS LANDED WHILE EN ROUTE BECAUSE OF LT MAGNETO FAILURE. IT WAS THEN DETERMINED NR 1 CYLINDER EXHAUST ROCKER CHAMBER WAS CRACKED. CYLINDER REPLACED AND AIRCRAFT CONTINUED FLIGHT.
