The pilot’s decision to takeoff from a contaminated runway with unfavorable wind conditions, and his failure to maintain airspeed during the initial climb, which resulted in an aerodynamic stall. Contributing to the accident was the operator’s inadequate procedures for operational control and dispatch.

The pilot's unfamiliarity with the airport environment, which resulted in his taxiing the airplane onto a no-taxi section of roadway and into a road sign. Contributing to the accident was the pilot's failure to avail himself of additional information that described the appropriate taxi routes on the airport.
The fracture of the airplane's nose gear scissor during landing, which resulted in a loss of control during the landing roll, and a nose down. A factor associated with the accident was the loss of nosewheel steering control.

REMOVED ALL HYDRAULIC LIFTERS TO INSPECT FOR SPALLING, DISCOVERED BADLY SPALLED EXHAUST LIFTER ON CYLINDER #4. INSPECTED CAM IN ACCORDANCE WITH CONTINENTAL SERVICE DIRECTIVE SID05-1B, INSTALLED NEW LIFTER AND WILL INSPECT CAM AGAIN IN 100 HOURS. IT IS DOUBTFUL THAT THIS 1900 HOUR ENGINE WILL MAKE TBO BEFORE NEEDING A NEW CAM. WE ARE NOW INSPECTING LIFTERS EVERY 200 HOURS.

PILOT REPORTED LOW OIL PRESSURE AT IDLE, REMOVED OIL PRESSURE REGULATOR AND FOUND SHARP METAL CHIP IN BOTTOM OF REGULATOR WHERE IT HAD ARRIVED AFTER FALLING OUT OF REGULATOR SEAT. PREVIOUS EXPERIENCE LED US TO BELIEVE THAT THE METAL WAS FROM A LIFTER, WE INSPECTED ALL 12 LIFTERS AND DISCOVERED A SEVERELY SPALLED EXHAUST LIFTER ON CYLINDER #4. INSPECTED CAM IN ACCORDANCE WITH CONTINENTAL SERVICE BULLETIN SID05-1B, INSTALLED NEW LIFTER AND WILL INSPECT CAM AND LIFTER AGAIN IN 100 HOURS (AS PER THE SID).

ON FIRST RUN UP OF THE DAY PILOT NOTED ACFT RAN ROUGH AND SOUNDED DIFFERENT. MX PERSONAL PERFORMED A VISUAL INSPECTION OF CYLINDERS AND FOUND NR 5 CYLINDER MISSING SEVERAL COOLING FINS AND A HOLE THE SIZE OF A PENCIL IN THE AREA OF THE EXHAUST PORT ON THE TOP OF THE CYLINDER HEAD. AFTER REMOVING CYLINDER A CRACK WAS NOTED FROM EXHAUST PORT ACROSS TOP SPARK PLUG BOSS TO INJECTOR PORT AND THOUGHT INTAKE PORT. ON THE INSIDE OF CYLINDER A CRACK PROPAGATED FROM TOP SPARK PLUG BOSS TO BOTH INTAKE AND EXHAUST SEATS. PN AEC63197, SN 20356-B, PART TT 1127.8, AD 2004-08-10 ADDRESSES SAME PN CYLINDER BUT LOWER SN THAN EFFECTED PART.
