The pilot's failure to maintain the proper glidepath during an instrument-landing-system (ILS) approach. Maintenance disclosure history

Service Difficulty Reports are self-reported maintenance findings. The NUMBER of reports is not a measure of reliability — diligent operators file more, not fewer. These are shown for transparency, not as a negative signal. To verify a report, search its control number at the FAA SDRS site.

Reports span 2014–2014. Most-reported systems: ATA 85 — 1. Most-common natures of condition, as filed: FLUID LOSS; VIBRATION/BUFFET — 1.

1 report on file.

Part: CYLINDER HEAD — CRACKED (ENGINE) · airframe 2,700 hrs · 2,000 cycles

WHILE CLIMBING THROUGH APPROXIMATELY 4,000 FT, HEARD AND FELT A SUDDEN VIBRATION. LOOKED TO THE LT ENGINE AND SAW THAT IT WAS VIBRATING MORE THAN NORMAL, AND THEN OIL BEGAN COMING OUT OF THE COWL. PLACED THE LT THROTTLE LEVER IN THE IDLE POSITION, FOLLOWED BY THE PROP LEVER TO THE FEATHER POSITION, THEN THE MIXTURE TO THE IDLE-CUT-OFF POSITION. THE ENGINE FEATHERED NORMALLY. DECLARED AND EMERGENCY, AND RETURNED TO DEPARTURE. SINGLE-ENGINE DESCENT, APPROACH AND LANDING WERE UNEVENTFUL. AFTER SHUTDOWN WE DE-COWLED THE ENGINE AND SAW THAT CYLINDER NR 3 HAD CRACKED COMPLETELY THROUGH AT THE HEAD, SEPARATING FROM THE CYLINDER BORE.
