Maintenance disclosure history

Service Difficulty Reports are self-reported maintenance findings. The NUMBER of reports is not a measure of reliability — diligent operators file more, not fewer. These are shown for transparency, not as a negative signal. To verify a report, search its control number at the FAA SDRS site.

Reports span 2024–2026. Most-reported systems: Main rotor drive (63) — 1; Rotorcraft flight controls (67) — 1. Most-common natures of condition, as filed: VIBRATION/BUFFET — 2.

2 reports on file.

Part: DRIVE SHAFT — CRACKED (GEARBOX) · airframe 14,139 hrs

DURING GROUND RUN A HIGH FREQUENCY VIBRATION WAS NOTICED BY THE PILOT AND THE AIRCRAFT WAS SHUTDOWN. DURING SHUTDOWN A MECHANIC NOTICED THE ANGLE GEARBOX (AGB) DRIVESHAFT APPEARED "BLURRY". UPON FURTHER INVESTIGATION IT WAS DISCOVERED THAT THE ANGLE GEARBOX INPUT FLANGE WAS CRACKED AT THE FLANGE TO FLEX COUPLING ATTACHMENT HOLE. THE AGB WAS REMOVED AND A SERVICABLE AGB WAS INSTALLED. AS A PRECAUTION THE DRIVE SHAFT ATTACHED TO THE INPUT FLANGE WAS ALSO REMOVED AND A SERVICABLE DRIVE SHAFT WAS INSTALLED. POST MAINTENANCE GROUND RUN DID NOT PRODUCE A VIBRATION AND THE AIRCRAFT WAS RETURNED TO SERVICE.
