DURING DEPARTURE FROM TUCSON INTERNATIONAL AIRPORT, (KTUS) IN N567MC FEB 8, 2026 AT 0900 LOCAL TIME, WHILE CLIMBING THROUGH APPROXIMATELY 23,000 FEET MSL, A LOUD AND ABNORMAL WHISTLING NOISE WAS NOTED FROM THE MAIN CABIN DOOR SEAL AREA. AIRCRAFT PRESSURIZATION INDICATIONS REMAINED STABLE AND WITHIN NORMAL LIMITS, WITH NO ABNORMAL CABIN ALTITUDE OR RATE OF CLIMB OBSERVED. THE DECISION WAS MADE TO DISCONTINUE THE CLIMB AND RETURN TO TUCSON. AIR TRAFFIC CONTROL WAS ADVISED, AND VECTORS WERE REQUESTED BACK TO TUS. THE RETURN WAS CONDUCTED AS A NON-EMERGENCY. THE AIRCRAFT LANDED WITHOUT INCIDENT AND FURTHER EVALUATION WAS CONDUCTED WITH THE DIRECTOR OF MAINTENANCE. FURTHER MAINTENACE INSPECTION OF MAIN ENTRANCE DOOR SEAL, FOUND DEBRIS LODGED BETWEEN INFLATABLE DOOR SEAL AND AIRFRAME.REMOVED DEBRIS AND AIRCRAFT DEPARTED WITHOUT ANY AIR LEAKS. THE DEBRIS WAS LOCATED UNDER THE FINAL STEP COVER WHEN STEPPING INTO THE CABIN.
LANDING RUNWAY 14 AT KBLM 1441 ZULU TIME ON SEPTEMBER 27, 2025. RIGHT MAIN TIRE BLOWOUT. CONDITIONS WERE VMC AND VISUAL FOR RUNWAY 14, AFTER TOUCHDOWN ACTIVATED THE SPEED BRAKES AND CAPTAIN ACTIVATED THE THRUST REVERSERS AT AROUND 90KTS, CALLED OUT 60KTS AND THE TRS BACK TO IDLE AND STOWED THE SPEED BRAKES WHICH THEN PASSING THE INTERCEPTING RUNWAY 03/21 AT AROUND 40-50KTS, WE NOTICED A SUDDEN DIP ON THE RIGHT SIDE OF THE AIRCRAFT AND A CHANGE OF âTAXI FEELINGâ CAPTAIN THEN CONTINUED TO TAXI DOWN TO CLEAR THE HOLD SHORT BARS BETWEEN RUNWAY 14/32 AND THE FBO. PRIOR TO THE FLIGHT, DURING PREFLIGHT AT DEPATURE AIRPORT, PERFORMED WALK AROUND AND NOTICED NO DAMAGES OR DEFECTS TO ALL TIRES/WHEEL/TREADS. AFTER WE CLEARED THE RUNWAY WE SHUT DOWN THE AIRCRAFT AND CONFIRMED THAT THE RIGHT MAIN TIRE WAS BLOWN.
DEPARTED KTYS AT 0745 LOCAL. RUNWAY 23L, WEATHER MOSTLY CLEAR WITH FEW SCATTERED CLOUDS. TEMP WAS 18C. WINDS 300 DEGREES AT 3KTS. TAKEOFF WEIGHT 16971LBS. FUEL AT TAKEOFF 4140 POUNDS OR 618 GALLONS. TAKEOFF WAS UNEVENTFUL. SKY CLEAR DURING CLIMB OUT. PASSING FL250 CAPTAIN HEARD LOUD THUMP, SUSPECTED BIRD STRIKE AT FIRST THEN THE LEFT AND RIGHT WINDSHIELD FAULT LIGHT ILLUMINATED FOLLOWED BY MASTER CAUTION LIGHT. CAPTAINS WINDSHIELD OUTER PANE THEN SHATTERED. CREW FOLLOWED EMERGENCY PROCEDURES CHECKLIST TURNING OFF WINDSHIELD HEAT SWITCHES. CAPTAIN DECLARED AN EMERGENCY WITH AIR TRAFFIC CONTROL AND A RETURN BACK TO DEPARTURE AIRFIELD (KTYS). LANDING WAS UNEVENTFUL.
ON A REPOSITION FLIGHT FROM KHYA TO KBED TO PICK UP PASSENGER. FLIGHT WAS OPERATED UNDER PART 91, NON-REVENUE AND NO PASSENGER. WEATHER AND VISIBILITY REPORTED BY KBEDâS TOWER AT MINIMUMS. CREWS PROCEEDED WITH FINAL APPROACH AND LANDED. DURING LANDING ROLLOUT, THE LEFT INBOARD FLAP STRUCK RUNWAY EDGE LIGHT. THE DAMAGES TO THE FLAP OCCURRED TWO INCHES FROM INBOARD EDGE AND FROM TRAILING EDGE UP TO CENTERLINE OF FLAP CHORD. CREWS DID NOT REPORT ANY DAMAGES TO THE FUSELAGE.
WITH NO PASSENGERS ON BOARD AND ON FINAL APPROACH TO OCEAN REEF CLUB AIRPORT (07FA) AFTER PLACING THE LANDING GEAR HANDLE IN THE DOWN POSITION THERE WAS AN UNSAFE INDICATION. TWO GREEN LIGHTS (NOT 3) AND ONE RED LIGHT ILLUMINATED. CYCLED THE GEAR ONE TIME AND THE SAME INDICATION APPEARED. FLEW AIRCRAFT TO (KOPF) OPA LOCKA AIRPORT. ASKED THE CONTROL TOWER ON A FLY-BY TO VERIFY THE GEAR WAS DOWN AND THEY AGREED IT APPEARED DOWN. LANDED SAFELY. AIRCRAFT WAS INSPECTED AND MAINTENANCE REPAIRS PERFORMED BY THE DIRECTOR OF MAINTENANCE. SIX LIGHT BULBS P/N 327 AND ONE GEAR MONITOR PCB P/N 6318394-1 BOARD WAS REPLACED. LANDING GEAR CHECK WAS PERFORMED AND AIRCRAFT WAS RETURNED TO SERVICE. WEATHER WAS CLEAR AND LIGHT WINDS.
AFTER TAKEOFF AND UPON PLACING THE GEAR TO THE UP POSITION THE HYD PRESS ON ANNUNCIATION REMAINED ON AND THE RED GEAR UNSAFE LIGHT ANNUNCIATION AND THE PUMP CONTINUED TO STAY RUNNING. FIRST OFFICER CALLED FOR THE HYD PRESS ON EMERGENCY PROCEDURES CHECKLIST. THE HYD PRESS ON WAS IDENTIFIED, CONFIRMED, AND CHECKLIST COMPLETED. THE CREW CYCLED THE LANDING GEAR TO THE DOWN POSITION. DURING TRANSIT THE CREWS IDENTIFIED AIRFLOW DISTURBANCES FOLLOWED BY THREE GREEN (NOSE, LH, RH) WITH THE RED GEAR UNSAFE LIGHT EXTINGUISHED AND DECIDED TO LEAVE THE GEAR IN THE DOWN POSITION AND FLAPS AT APPROACH. THE CAPTAIN ELECTED TO BE CAUTIOUS AND INFORMED ATC THAT THEY WOULD RETURN TO KAPA AND PERFORMED ONE LOW APPROACH OVER THE RUNWAY FOR TOWER TO GIVE A VISUAL LOOK, AND THE CONTROLLER REPORTED N414FW YOUR LANDING GEAR APPEARS TO BE DOWN. THE CAPTAIN DECLARED AN EMERGENCY FOR WHAT APPEARED TO BE AN ABNORMAL CHECKLIST ITEM AND LANDED WITHOUT INCIDENT.
DEPARTED IN ROUTE. AFTER POSITIVE RATE OF CLIMB AND ABOUT 300 FEET AGL GEAR HANDLE UP. BOTH MLG RETRACTED, NOSE GEAR DID NOT. THE NOSE GEAR DOWN AND LOCKED GREEN LIGHT WAS ILLUMINATED, NO RED UNLOCKED LIGHT AND WIND NOISE FROM THE OPEN NOSE GEAR DOORS WAS PRESENT. FO COMPLETED AFTER TAKEOFF CHECKLIST AND AT 1000 FEET MSL AND PROMPTLY LOCATED APPROPRIATE ABNORMAL CHECKLIST FOR THE NOSE GEAR PROBLEM. THE CREW THOROUGHLY COMPLETED THAT CHECKLIST. THE CAPTAIN CONTACTED ATC AND ASKED FOR A HEADING AND ALTITUDE TO WORK THE PROBLEM. THE NOSE GEAR WAS NOT GOING TO RETRACT SO ASKED FOR HEADINGS TO RETURN TO DEPARTURE. PERFORMED THE DESCENT, APPROACH AND BEFORE LANDING CHECKLISTS AND LANDED ON RUNWAY 27R WITHOUT INCIDENT. WE HAD 3 DOWN AND LOCKED GREEN LIGHTS AND NO RED UNLOCKED LIGHT. ALSO, NO GEAR WARNING HORN ON FINAL WITH FLAPS AT 35 DEGREES. NO EMERGENCY WAS REPORTED TO ATC DURING THE INCIDENT AND NO EMERGENCY WAS REQUESTED DURING THE APPROACH OR LANDING.
HYDRAULIC FLUID NOTED IN AFT EQUIPMENT BAY. FOUND LANDING GEAR CONTROL TUBING HAD BEEN CHAFED BY AIR CYCLE MACHINE INLET FLEX DUCT.
