BAFFLING THAT SEPARATES THE OIL COOLER AND THE NR 2 CYLINDER, CRACKED AND SEPARATED. IT CAME TO REST ON THE STARTER POSITIVE TERMINAL AND NR 2 CYLINDER ROCKER BOX OIL RETURN LINE, PN 72703. WHEN THE STARTER WAS ENGAGED THE ELECTRICAL CURRENT ARCED THROUGH THE SHEET METAL BAFFLE TO THE OIL RETURN LINE SEVERING THE ALUMINUM TUBING. LOSS OF ENGINE OIL PRESSURE OCCURRED DURING START ACCOMPANIED BY SMOKE. OIL SPILLED TO THE GROUND BELOW THE ACFT.
CLUTCH ACTUATOR MOTOR FAILED. (4TH TIME)
BELTS WERE REPLACED DURING 100HR INSPECTION AFTER 693.3 HOURS TSI. FAN WHEEL WAS BALANCED AND RUNUP WAS NORMAL. FIRST FLIGHT OF APPROXIMATELY 2 HOURS WAS UNEVENTFUL. DURING SECOND FLIGHT PILOT REPORTED CLUTCH LIGHT FLICKERED ON AND OFF SEVERAL TIMES. ON INSPECTION AFTER PRECAUTIONARY LANDING, BELTS WERE FOUND STRETCHED TO THE POINT THAT OVER LIMIT SWITCH DEACTIVATED THE SYSTEM. SYSTEM INSPECTED WITH NO FAULTS FOUND OTHER THAN BELT STRETCH BEYOND LIMITS. NEW BELTS WERE INSTALLED AND ADJUSTED. FAN WHEEL BALANCE CHECKED.
GOVERNOR MOTOR RUNS SLOWLY, TAKING 10-12 SECONDS TO BRING RPM UP TO FLIGHT RPM REPLACEMENT MOTOR INSTALLED, DEFECTIVE UNIT RETURNED TO ROBINSON FOR WARRANTY EXCHANGE OR REPAIR.
PAINT ON FAN WHEEL LOOSE AND FLAKING OFF DUE TO POOR ADHESION, PROBABLY CAUSED BY IMPROPER PREPARATION, ALLOWING PREMATURE CORROSION.
BLADE REMOVED FOR REPAIR OF LEADING EDGE ABRASION STRIP CRACKS. CUSTOMER REQUESTED EXCHANGE BLADE IN ORDER TO RESUME FLIGHTS. EXCHANGE BLADE INSTALLED, REPAIRABLE BLADE RETURNED TO AEC
CLUTCH ACTUATOR MOTOR FAILED FOR THE 2ND TIME. REPLACEMENT MOTOR INSTALLED. FURTHER TOUBLESHOOTING REVEALED SHEAVE ALIGNMENT OUT OF LIMITS CAUSING INFLIGHT ILLUMINATION OF CLUTCH LIGHT. ALIGNMENT CORRECTED AND FLIGHTS CONTINUED NORMALLY. BAD MOTOR RETURNED FOR WARRANTY CREDIT.
MOTOR FAILED TO OPERATE WHEN CALLED ON TO ENGAGE M/R DRIVE BELTS. WOULD NOT OPERATE IN EITHER DIRECTION. MOTOR REPLACED, DEFECTIVE MOTOR RETURNED TOROBINSON FOR WARRANTY CREDIT.
PILOT REPORTED ANTI COLLISION LIGHT INOP. FLASH TUBE FOUND BURNED AND CRACKED. REPLACEMENT TUBE ORDERED AND INSTALLED. OPS CHECKED OK.
THE FIRST CLASS AND FWD SECTION OF COACH HAD EGRESS LIGHTING INOPERATIVE. FOUND POWER FEEDER LEAD LOOSE. RESECURED CONNECTOR. CHECK-C 3-12-98 STL.

STL - FLT 619 - DURING PRE-FLIGHT, EMERGENCY LIGHTS AND EGRESS LIGHTS WOULD NOT ILLUMINATE. FOUND SWITCH HAD BEEN LEFT IN THE ON POSITION. REPLACED BATTERY POWER PACKS, MFG PN 5822-2 AND VR4D. EMERGENCY LIGHTS AND EGRESS LIGHT SYSTEM OPERATION CHECKED NORMAL. CHECK C - 5-22-95 - STL. (X)
