DUCT TEMP OVERHEAT ANNUNCIATOR FOLLOWED BY SMOKE REPORTED IN CABIN. PERFORMED GROUND RUNS FOR TROUBLESHOOTING AND DUPLICATED PROBLEM. LOWER COOLING TURBINE PASSENGER LOWER COOLING TURBINE PRODUCING A DUCT OVER HEAT AND SMOKE IN CABIN. SUSPECT TURBO COOLER BEARING FAILURE.
ENGINE OIL PRESSURE CLIMBED INTO RED ZONE ON INDICATOR AND WOULD NOT RESPOND TO THROTTLE CHANGES. PAX REPORTED EVIDENCE OF SMOKE IN THE CABIN. CREW SHUT DOWN ENGINE AND DIVERTED TO KACY. WE SUSPECT OIL PUMP MAY BE THE CULPRIT FOR HIGH OIL PRESSURE.
ON FINAL APPROACH LANDING GEAR WOULD NOT EXTEND. CREW HAD TO USE EMERGENCY GEAR EXTENSION PROCEDURES TO GET GEAR EXTENDED. AIRCRAFT LANDED WITH NO FURTHER ISSUES. AIRCRAFT IS OUT OF SERVICE AND IN MX FOR TROUBLESHOOTING.
DURING CRUISE FLIGHT AT AN ALTITUDE OF FL380 ENROUTE FROM TORONTO-PEARSON INTERNATIONAL AIRPORT (CYYZ) TO MCCARREN INTERNATIONAL AIRPORT (KLAS) THE CREW EXPERIENCED INTERMITTENT FOLLOWED BY CONTINUOUS TO MODERATE VIBRATION ACCOMPANIED BY A LOW FREQUENT "HUMMING" SOUND. HAVING NO ABNORMAL ENGINE INDICATIONS, THE CREW CONCLUDED THAT THE ISSUE WAS BEING CAUSED BY EITHER AN ALTERNATOR OR STARTER/GENERATOR FAILING. THE CREW PROCEEDED TO DIVERT TO DENVER CENTENNIAL AIRPORT (KAPA) WITHOUT INCIDENT.
DURING THE CLIMB, CREW REPORTED LOUD SQUEAL FOLLOWED BY ODOR OF BURNED OIL AND REPORT OF SMOKE COMING FROM AIR VENTS. THE ACM, COOLING TURBINE FOUND TO HAVE FERROUS MATERIAL ON THE MAGNETIC DRAIN PLUG, COOLING TURBINE WAS DISCOVERED TO BE FAILED, INTERNAL BEARING FAILURE CAUSING UNIT TO LOCK UP.
AFTER DEPARTURE, WHILE IN THE CLIMB, ACFT EXPERIENCED SMOKE COMING FROM THE RT ENGINE COWL LOUVER DOORS AND VISIBLY HAD OIL EXITING THE ENGINE COWL UPPER DOOR (OIL FILL ACCESS) AND THE INBD SIDE OF THE ENGINE COWLS. THE ENGINE WAS SECURED AND THE ACFT RETURNED TO DEPARTURE, AFTER LANDING AND WHILE THE ACFT WAS TAXING ONTO THE RAMP THE OIL THAT HAD EXITED THE ENGINE AND COLLECTED ON THE TURBO-CHARGER HEAT SHIELD (BLANKET) IGNITED AS THE ACFT CAME TO A STOP AND WAS RAPIDLY EXTINGUISHED BY THE DOM. UPON INSPECTION OF THE ENGINE IT WAS DISCOVERED THAT THE OIL DIPSTICK HAD NOT BEEN SECURED ALLOWING OIL TO EXIT THE ENGINE OUT THE OIL FILL TUBE. DIPSTICK WAS NOT PROPERLY SECURED BY THE LINE SERVICE PERSONNEL AND (PIC) AFTER IT WAS SERVICED. TRAINING REVIEW TO VERIFY THAT THE DIPSTICK IS PROPERLY SECURED WITH BOTH THE LINE SERVICE PERSONNEL AND THE PILOT INVOLVED.
