ON TWO SEPARATE OCCASIONS, ON THE AS350B3, 6EA MAIN ROTOR STARFLEX ATTACHMENT BOLTS WERE FOUND TO BE UNSERVICEABLE. NOTE: THE STARFLEX IS ATTACHED TO THE MAIN ROTOR SHAFT USING 12EA BOLTS AND 12EA BARREL NUTS.
DURING INSPECTION FOUND RIGHT HAND MAIN ROTOR SERVO HAS STATIC LEAK. REPLACED SERVO.
DURING LANDING AND TAXI, CREW NOTICED AN INTERMITTENT NOISE COMING FROM RT MLG AREA AND TIRE APPEARED TO BE LOW. ACFT FOUND WITH A FLAT TIRE AND ONCE HUB CAP REMOVED, NUTS AND WASHERS FROM WHEEL ASSY WERE FOUND LOOSE AND SOME MISSING. WHEEL REMOVED AND SEVERAL OF THE WHEEL HALF BOLTS WERE FOUND TO HAVE CONTACTED BRAKE ASSY AS WHEEL ROTATED. WHEEL HAD ONLY BEEN INSTALLED ON THIS ACFT FOR A TOTAL OF 10.1 HRS AND 14 LANDINGS. WHEEL TSO IS 10.1 HRS/14 LANDINGS. WHEEL OVERHAULED FOR EVALUATION AND INVESTIGATION.

THE ACFT WAS FOUND WITH A FLAT TIRE AND ONCE THE HUB CAP WAS REMOVED, SOME NUTS AND WASHERS FROM THE WHEEL ASSY WERE FOUND LOOSE AND SOME MISSING. THE WHEEL WAS REMOVED AND SEVERAL OF THE WHEEL HALF BOLTS WERE FOUND TO HAVE CONTACTED THE BRAKE ASSY AS THE WHEEL ROTATED. THE WHEEL HAD ONLY BEEN INSTALLED ON THIS ACFT FOR A TOTAL OF 10.1 HRS AND 14 LANDINGS. THE WHEEL TSO IS 10.1 HRS/14 LANDINGS. THE WHEEL WAS RETURNED TO THE REPAIR STATION WHERE IT WAS O/H FOR EVALUATION ADN INVESTIGATION.
SMOKING AT LT WING FAIRING EDGES. FAIRING PULLED, FOUND HYD PKG SOFT START RESISTOR SMOKED AS WELL AS LARGE BLACK AREA ON FUSELAGE BLACK. INSIDE OF WING FAIRING BURNED AND BUBBLED ON OUTSIDE. INSTALLED NEW RESISTOR AND NEW HYD UNIT SELECT RELAY. PULLED NR 4 CABIN CENTER LINE FLOOR. FOUND HYD UNIT 100-AMP CB POPPED. 100 AMP CB RESET. BATT SELECTED ON. PUMP DID NOT RUN AND RESISTOR STARTED HEATING. BATT OFF. CALLED TECH SUPPORT. HYD PKG AND ACCUMULATOR PULLED. CLEANED SIDE OF FUSELAGE AND FOUND ONLY SLIGHT DISCOLORATION TO PAINT. NO DAMAGE TO SKIN. REPLACED HYD CNTRL RELAY, HYD PKG, ACCUMULATOR, SOFT START RESISTOR, COMPLIED WITH SEVERAL GEAR SWINGS, NO PROBLEM NOTED.
