Bell 407 (N191SF) Fatal injury Part 135

Operated by VIKING AVIATION INC (per NTSB report)

The NTSB determines that the probable cause of this accident was Survival Flight's inadequate management of safety, which normalized pilots' and operations control specialists' noncompliance with risk analysis procedures and resulted in the initiation of the flight without a comprehensive preflight weather evaluation, leading to the pilot's inadvertent encounter with instrument meteorological conditions, failure to maintain altitude, and subsequent collision with terrain. Contributing to the accident was the Federal Aviation Administration's inadequate oversight of the operator's risk management program and failure to require Title 14 Code of Federal Regulations Part 135 operators to establish safety management system programs.

Maintenance disclosure history

Service Difficulty Reports are self-reported maintenance findings. The NUMBER of reports is not a measure of reliability — diligent operators file more, not fewer. These are shown for transparency, not as a negative signal. To verify a report, search its control number at the FAA SDRS site.

Reports span 2013–2025. Most-reported systems: Fuselage (53) — 3; Rotorcraft flight controls (67) — 2; Engine (72) — 2; Servicing (12) — 1; ATA 14 — 1 (top 5 of 10 systems, covering 9 of 14 coded reports). Most-common natures of condition, as filed: OTHER — 7; WARNING INDICATION — 3; FLT CONT AFFECTED — 1; INADEQUATE Q C — 1; SMOKE/FUMES/ODORS/SPARKS — 1 (top 5 of 6 conditions, covering 13 of 14 coded reports).

14 reports on file.
