DURING AN OIL CHANGE FLAKES OF METAL WERE FOUND IN THE OIL FILTER. THE TOP SPARK PLUGS WERE REMOVED & CYLINDER NR 3 HAD OIL ON THE TOP SPARK PLUG. THE CYLINDER WAS REMOVED & PLATING WAS FOUND TO BE FLAKING OFF THE TOP PISTON RING. ALL 6 CYLINDERS WERE REMOVED & ALL TOP RINGS HAD PLATING STARTING TO FLAKE OFF TO SOME DEGREE. ALL CYLINDERS, PISTONS & RINGS HAVE BEEN SENT TO THE MANUFACTURE FOR INSPECTION & REPAIR.
AFTER REMOVAL OF ALL CYLINDERS DUE TO TOP PISTON RING PLATING FLAKING, SDR A6YA2010052738221, ALL LIFTERS WERE REMOVED & INSPECTED IAW SID 05-1A. NR 2 & NR 5 EXHAUST VLV LIFTERS WERE FOUND TO HAVE FULL FACE SPALLING AS PICTURED IN SID 05-1A FIG 6 AND FIG 8. ALL OTHER EXHAUST LIFTERS HAD CORROSION PITTING TO SOME DEGREE. ALL LIFTERS WERE REMOVED & SENT TO THE MANUFACTURE FOR INSPECTION. THE CAMSHAFT WAS INSPECTED IAW SID 05-1A & FOUND TO BE IN SERVICEABLE CONDITION. THIS ENG WAS INSTALLED ON 1-25-2012 IN NEW CONDITION.
THE FLIGHT CREW CALLED & STATED THE RT ENG WAS MAKING A WHINING SOUND & THERE WAS AN ODOR JUST AFTER LANDING. ALL IN FLIGHT ENG INDICATIONS WERE REPORTED AS NORMAL. THE FLIGHT CREW WAS ASKED TO START THE RT ENG. THE GND IDLE WAS OBSERVED FLUCTUATING FROM 54% TO 56% N1 WITH INCREASE IN ITT FROM 660C TO 720C AT 4 SECOND INTERVALS. TROUBLESHOOTING WHEN THE RT ENG COWLING WAS OPENED A HOLE THROUGH COMPRESSOR INLET SCREEN WAS FOUND. A SINGLE FIRST STAGE COMPRESSOR BLADE WAS IN THE BOTTOM OF THE COWLING. THE ENG IS BEING REMOVED & WILL BE SENT TO AN OVERHAUL FACILITY FOR EVALUATION.
DURING CLIMB AT APPROXIMATELY 20,000 FT. REAR BAY OVERHEAT INDICATOR ILLUMINATED, EMERGENCY PROCEDURES FOLLOWED AND LANDED WITH OUT INCIDENT. A BLEED AIR LEAK WAS FOUND BETWEEN FLOW CONTROL VALVE AND BLEED AIR DISTRIBUTOR DUCT. V BAND CLAMP WAS REPOSITIONED AND DUCT, RESEATED. A LEAK CHECK WAS PERFORMED WITH NO LEAKS NOTED. THE ACFT CONTINUED ON TO ITS FINAL DESTINATION WITH NO FURTHER PROBLEMS.
APPROXIMATELY 20 MINUTES INTO THE FLIGHT PILOT SUSPECTED AN ELECTRICAL BURNING SMELL. HE NOTICED SOME SMOKE COMING FROM UNDER INSTRUMENT PANEL. AT THAT TIME HE COMPLETED THE MEMORY ITEMS FOR (ELECTRICAL SMOKE OR FIRE). AFTER 1 TO 2 MINUTES SMOKE AND SMELL DISSIPATED. AFTER REGAINING RADIO COMMUNICATIONS WITH MINNEAPOLIS CENTER PILOT DECLARED AN EMERGENCY AND ASKED FOR VECTORS TO NEAREST AIRPORT. PILOT ACCEPTED VECTORS. FLIGHT LANDED UNEVENTFULLY. NO EMERGENCY EVAC, RADIO COOLING FAN FOUND RUBBING BETWEEN FAN AND HOUSING, REPOSITIONED FAN. INSPECTED SIMILAR COOLING FANS ACROSS FLEET. (K)
