INSTALLED ENGINE AFTER OVERHAUL. AFTER THIRD START, THE PROPELLER FEATHERED. FOUND OIL TRANSFER COLLAR SPALLED/GROOVED. ENGINE HAD LESS THAN 3 MINUTES TOTAL TIME SINCE OVERHAUL WHEN THE PROPELLER FEATHERED.
FOUND #1 (S/N: 65688-04), #3 (S/N: 65976-01), #4 (S/N: 65937-18), #5 (S/N: 66237-24) AND #6 (S/N: 65025-20) CYLINDERS CRACKED AT CYLINDER HEAD ON LEFT ENGINE DURING 100 HOUR INSPECTION.
FOUND #2 (S/N: 65537-01), #3 (S/N: 65656-01), #5 (S/N: 65537-08) AND #6 (S/N: 65622-07) CYLINDERS CRACKED AT CYLINDER HEAD ON RIGHT ENGINE DURING 100 HOUR INSPECTION.
SPALLED CAMSHAFT.
LEFT ENGINE HAS SPALLED INTAKE LIFTERS.
LEFT ENGINE HAS SPALLED EXHAUST LIFTERS.
RIGHT ENGINE HAS SPALLED CAMSHAFT.
FOUND SPALLED INTAKE LIFTER ON RIGHT HAND ENGINE.
FOUND SPALLED EXHAUST LIFTER ON RIGHT HAND ENGINE.
DURING GROUND OPERATION, CAM SHAFT GEAR BROKE TWO TEETH. MOTOR AND DIRECTLY RELATED ACCESSORIES WERE REMOVED AND O/H. IT WAS UNDETERMINED BY THE O/H SHOP, THE CAUSE OF THE GEAR FAILURE.
LT MLG ACTUATOR WAS LEAKING OUT PISTON SEAL DUE TO NORMAL AGE AND USE.
FOUND LT ENGINE NR 6 CYLINDER CRACKED IN INTAKE SIDE OF HEAD DURING 100 HR INSPECTION.
ENGINE DIED ON INITIAL RUN-UP DUE TO FUEL STARVATION. FOUND GALLING BETWEEN FUEL PUMP DRIVE GEAR AND HOUSING. CAUSE UNDETERMINED. SENT BOTH PARTS OUT FOR O/H.
DURING AN ANNUAL/100HR INSP FOUND 4 OUT OF THE 6 CYLINDER HEADS CRACKED, THEY ARE ECI CYLINDERS WITH PN TIST71.4, SN'S 38519-08, 38542-17, 38947-18, 38947-30.
THERE WAS NO (GEAR DOWN) INDICATION WHEN THE LANDING GEAR WAS EXTENDED. PILOT DECLARED EMERGENCY AND DIVERTED TO AN AIRPORT WITH A LONGER RUNWAY. LANDING WAS NORMAL AND UNEVENTFUL. UPON EXAMINATION IT WAS FOUND THAT THE BRACKET HOLDING THE NOSE GEAR (DOWN) SQUAT SWITCH HAD BROKEN AND WAS DANGLING. (K)
