Maintenance disclosure history
Service Difficulty Reports are self-reported maintenance findings. The NUMBER of reports is not a measure of reliability — diligent operators file more, not fewer. These are shown for transparency, not as a negative signal. To verify a report, search its control number at the FAA SDRS site.
Reports span 2017–2020. Most-reported systems: Engine fuel & control (73) — 2; Tail rotor (64) — 1; Rotorcraft flight controls (67) — 1. Most-common natures of condition, as filed: OTHER — 2; PARTIAL RPM/PWR LOSS — 1; WARNING INDICATION — 1.

Part: BEARING — SEIZED (T/R SWASHPLATE)
PILOT COMPLAINED OF STICKY T/R PEDAL MOVEMENT, FOUND T/R SWASHPLATE PIVOT BEARING SEIZED. REMOVED T/R SWASHPLATE AND SENT IT FOR REPAIR, INSTALLED SERVIVEABLE UNIT. WE HAVE 7 INCIDENTS IN THE LAST YEAR WITH THESE BEARINGS 369A7951-13 SEIZING, ALL UNDER 200 HOURS TSO.
Part: BEARING — SEIZED (MIXER ASSY)
PILOT NOTICED THE COLLECTIVE MOTION WAS RATCHETY. INSPECTION FOUND THAT THE BEARINGS, SELF-ALIGNING, WERE SEIZED UP SOLID. REPLACEMENT OF THE LINK ASSY CURED THE ISSUE. THE LINK HAD BEEN INSTALLED NEW, 3 DAYS BEFORE WITH 11.4 HOURS TSN AT REMOVAL.
Part: GOVERNOR — FAILED (ENGINE)
LOW SIDE GOVERNOR FAILURE, EPO ILLUMINATED, N1, N2, TORQUE DROP INDICATOR LOSS OF POWER. PILOT LANDED IMMEDIATELY WITHOUT INCIDENT. UPON POWER DOWN THE PILOT LOWERED THE COLLECTIVE AND THE RPM SEEMED TO SPOOL UP AND FLUCTUATE. PILOT ROLLED THROTTLE TO IDLE AND FLUCTUATED AT IDLE AS WELL. PILOT SHUT THE ENGINE DOWN. REPLACEMENT OF THE GOVERNOR REPAIRED THE PROBLEM. THIS UNIT HAD GONE IN FOR REPAIR 3 TIMES BEFORE THE RPM AND ADJUSTMENT ISSUES. TSO 1372.6 TREM 627.4
Part: FUEL CONTROL — FAILED (ENGINE)
PILOT EXPERIENCED DELAYED HOT AND ERRATIC STARTS. FOUND PLAY IN THE POINTER ROD, THIS WOULD AFFECT THE START SCHEDULING. THE SLOP IS WHERE THE POINTER ROD ENGAGES THE THROTTLE ARM. AT TIMES THE THROTTLE SEEMED AS IF IT WERE HITTING A STOP. FIRST START OK, SECOND START AS THE ENGINE REACHED IDLE IT SUDDENLY ACCELERATED. THE PILOT HAD TO USE MORE THAN NORMAL FORCE TO SHUT OFF THE THROTTLE. REPLACED FCU. THIS OUR 4TH SUCH INCIDENT.
