Maintenance disclosures
WHILE IN CRUISE FLIGHT AT 10,000FT PILOT BEGAN TO SMELL SOMETHING SUSPICIOUS. MOMENTS LATER THE PILOT EXPERIENCED A LEFT AC BUS FAILURE AND BEGAN TO SEE SMOKE COMING FROM UNDERNEATH THE CO-PILOTS SIDE OF THE AVIONICS PANEL. IMMEDIATELY PROCEEDED WITH THE MEMORY ITEM OF TURNING OFF THE LEFT AC BUS INVERTER. THE PILOT BEGAN TO STABILIZE THE AIRCRAFT USING STANDBY INSTRUMENTS AND SHORTLY AFTER DECLARED AN EMERGENCY. THEPILOT ALSO DONNED MY 02 MASK WHILE RUNNING THE QRH FOR THE AC BUS FAILURE AND RECEIVING VECTORS FOR THE ILS RWY32 INTO KHUF. UPON LANDING THE PILOT SAFELY VACATED THE RUNWAY AND IMMEDIATELY SHUT DOWN THE AIRCRAFT ON A TAXIWAY.
SIG EVENT, PILOT REPORTED THAT THERE WAS A STRONG SMELL OF FUEL IN CABIN. REMOVED FLOORBOARDS & INSPECTED FUEL TRANSFER LINES, PRESSURIZED & LEAK CHECKED FUEL LINES, NO DISCREPANCIES NOTED. INOPERATIVE LT & RT ENG ACCESSORIES FOR FUEL LEAKS, NO LEAKS NOTED ATT. STARTED ENGS & PERFORMED CHECKS, NO FUEL SMELL IN THE BLEED AIR, SHUTDOWN ENGS & INSPECTED LT & RT FUEL NOZZLES & FLOW DIVIDERS FOR FUEL LEAKS, NO LEAKS NOTED. INSPECTED LT & RT PURGE CANS FUEL CHECK VLVS, ONE OF THE LT ENG PURGE CAN CHECK VLVS NOT HOLDING. R & R CHECK VLV, CLEANED CABIN SEATS, SEAT BACKS & ARM RESTS. ALSO AERATED & DEODORIZED CABIN CARPETS, RETURNED ACFT TO AIRWORTHY CONDITION, NO MORE FUEL SMELL NOTED ATT.
PILOT REPORTED THAT THERE WAS A STRONG BURNING SMELL COMING FROM THE CABIN & IS IRRITATING TO TO EYES. TROUBLESHOT BURNING SMELL COMING FROM FLAP GEAR BOX MOTOR ASSEMBLY. R & R FLAP GEAR BOX MOTOR ASSEMBLY PN 100-524073-1 SN OFF 5693, IAW BMM 27-50-03 OPERATIONS CHECKED GOOD.
PILOT REPORTED THAT WHEN THEY WENT TO EXTEND THE FLAPS HAD A STRONG ELECTRICAL SMELL & THE FLAPS DID NOT EXTEND. MAINTENANCE R&R FLAP GEAR BOX MOTOR P/N 100-524073-1 S/N OFF 6137 S/N ON. 5693 OPS CHECK GOOD AT THIS TIME REF 1900DMM 27-50-03.
DURING A SCHEDULED INSPECTION, FOUND THE LT LOWER BELLY SKIN AT FS 288 CRACKED, 1.75" IN LENGTH. REPAIRED IAW EO 13-1900-53-059. MAJOR REPAIR.
SIG EVENT - PILOT REPORTED THAT THE RT FIREWALL FUEL VALVE T-HANDLE FLICKERED ON AND OFF IN FLIGHT. MX TESTED FIRE LOOP ON RT SIDE, TESTED NORMAL. INSPECTED FWD AND AFT LOOP FOUND CHAFE ON TOP SIDE OF FWD LOOP. R & R LOOP IAW BMM 26-10-01. OPS CHECK GOOD AT THIS TIME PN 244-12886.

DURING A SCHEDULED ROUTINE INSPECTION, FOUND 2 CRACKS AT THE CARGO DOOR FUSELAGE FRAME AT FS 509 BETWEEN STRINGER 4L & 5L, LOCATED IN THE BEND RADIUS OF THE FRAME, 1" IN LENGTH. REPAIRED IAW EO 13-1900-53-042. MAJOR REPAIR.
DURING A SCHEDULED ROUTINE INSPECTION, FOUND A CRACK IN THE TEE SECTION OF THE LT HORIZONTAL STAB LOWER AT HSS 5.000 AND 10.5 PERCENT AT A SCREW HOLE, .5" IN LENGTH. REPAIRED IAW EO 13-1900-55-011. MAJOR REPAIR.
DURING A SCHEDULED ROUTINE INSPECTION FOUND A CRACK EXTENDING FROM A SCREW HOLE ON THE TOP SIDE OF THE LT HORIZONTAL STAB AT HSS 5.000 AND 10.5 PERCENT CHORD, ON THE FLANGE FOR PANEL 332 BTC ON THE FORWARD LT CORNER, .25" IN LENGTH. REPAIRED IAW EO 13-1900-55-009. MAJOR REPAIR.

DURING A SCHEDULED ROUTINE INSPECTION, FOUND A CRACK AT THE NUTPLATE, FWD CARGO DOOR SNUBBER FUSELAGE ATTACH POINT, UPPER IB NUTPLATE FS 456, .35" IN LENGTH. REPAIRED IAW EO 12-1900-53-014. MAJOR REPAIR
PILOT REPORTED THAT THE GEAR INTRANSIT LIGHT ILLUMINATED IN FLIGHT, RAN CHECKLIST, GEAR OPERATED NORMALLY TO THE EXTENDED POSITION, EMERGENCY DECLARED, LANDED SAFELY. MX COULD NOT DUPLICATE, REPLACED THE NLG UPLOCK SWITCH IAW TC 1900-3260.00, OPS CHECKED GOOD.
PILOT REPORTED THAT THE LANDING GEAR DID NOT EXTEND WHEN SELECTED AND RELAY CIRCUIT BREAKER TRIPPED, MANUALLY EXTENDED GEAR, NO IN-TRANSIT LIGHTS, LANDED SAFELY. MX REPLACED THE PCB A116 AND THE LANDING GEAR RELAY, OPS CK GOOD.
PILOT REPORTED THAT THE LEFT GEAR INDICATION DID NOT SHOW DOWN AND LOCKED UNTIL APPROX 25 SECONDS AFTER THE HANGLE WAS PLACED IN THE DOWN POSITION. MX REPLACED THE LT MLG ACTUATOR IAW TC 1900-3230.10, PN: 114-300041-15, SN: 0561A, OPS CK GOOD.
PILOT REPORTED THAT THE ACFT YAWED TO THE LT IN THE LANDING FLARE. MX CLEANED THE LOW PITCH SOLENOID, OPS CHECKED GOOD.
PILOT REPORTED THAT THE CABIN DOOR ANNUNCIATOR REMAINS EXTINGUISHED WHEN THE DOOR IS OPENED UP UNTIL ABOUT A FOOT OPEN. MX INSPECTED WIRE AT AIRSTAIR DOOR FOUND 4 CHAFED WIRES, REPAIRED WIRE. PERFORMED CABIN DOOR AM LIGHTING SYS CK, OPS CK GOOD REF CHAP 14, SECT 14.3.0 & REF 52-70-00.
PILOT REPORTED THAT THE CABIN DOOR ANNUNCIATOR GOES OUT WITHOUT LOCKING THE DOOR. MX CLEANED THE MICRO SWITCHES, OPS CK GOOD.
PILOT REPORTED THAT THE CABIN DOOR ANNUNCIATOR WENT OUT BEFORE THE DOOR WAS LOCKED. MX CLEANED THE CABIN DOOR MICRO SWITCH REF AIM 14.3.0, OK FOR SERVICE.
PILOT REPORTED THAT THE LT MLG BOTH GREEN "L" AND "H" ANNUNCIATORS FLICKERED THEN EXTINGUISHED THEN REILLUMINATED, NO UNSAFE INDICATIONS ON APPROACH & LANDING. MX REPLACED THE LT MLG ACTUATOR, OPS AND RIG CKS IAW TC. 1900-3230.10.
PILOT REPORTED THAT UPON RAISING THE LANDING GEAR WHILE FLAPS AT 17 DEGREES. TAKEOFF GEAR WARNING HORN SOUNDED AND COULD NOT BE SILENCED. IN TRANSIT LIGHT REMAINED ILLUMINATED UNTIL FLAPS 0 DEGREES WAS OBTAINED. MX RIGGED LANDING GEAR WARNING HORN FLAP POSITION SWITCH IAW MM 27-50-07.
PILOT REPORTED THAT WITH THE CABIN DOOR OPEN THE CABIN ANNC DID NOT ILLUMINATE. MX CLEANED THE MICRO SWITCH, SYS OPS CK GOOD.
MAJOR REPAIR - DURING A SCHEDULED ROUTINE INSPECTION, MX FOUND A GOUGE IN THE RT OTBD KEEL BY A CLAMP ON THE RETRACT/EXTEND LINE FOR THE LT MLG ACTUATOR. MX REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 11-1900-57-006. MATERIALS UTILIZED ARE INCLUDED IN THE EO.
MAJOR REPAIR - DURING A SCHEDULED ROUTINE INSPECTION, MX FOUND A GOUGE IN THE LT OTBD KEEL BY A CLAMP ON THE RETRACT/EXTEND LINE FOR THE LT MLG ACTUATOR. MX REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 11-1900-57-005. MATERIALS UTILIZED ARE INCLUDED IN THE EO.
CLIMBING THROUGH 20,000 FT COULDN'T GET CABIN TO QUIT CLIMBING, HAD TO DEPLOY MASKS & RETURN TO DEPARTURE, DIR STUCK AT 3.4 PSI WOULD NOT GO HIGHER. STOWED & RELOCKED MASKS IAW MM 35-20-01. RAN ENG & PRESSURIZED - NO AIR FROM LT BLEED, REMOVED ORIFICE PLATE FROM BLEED AIR DUCT, RAN ENGINES PER TC 1900-7100.10, SYS OPS CK GOOD IAW BMM 21-30-00.
SIG EVENT - PILOT REPORTED THAT AN OVERWHELMING ODOR OF FUEL ENTER THE CABIN AND COCKPIT. MAINTENANCE FOUND LOOSE FASTENERS JUST OUTBOARD OF RT NACELLE AT FORWARD SPAR, REPLACED FASTENERS AND SEALED AREA, REFUELED, NO LEAKS OR ODOR PRESENT IAW SRM 51-10-00, CHECKS GOOD PER BMM 28-10-00 (1)(A).
OVERWHELMING ODORS OF JET FUEL PRESENT IN CABIN COCKPIT EXTREMELY BAD TO A POINT OF BEING NAUSEATING AND AFFECTING SAFETY SLOW DRIP UNDER RT NACELLE DIFFERENT AREA THAN REPAIR.
DURING A SCHEDULED ROUTINE INSPECTION A CRACK WAS FOUND AT THE AFT CARGO BAY FLOORBOARD FORWARD SUPPORT BULKHEAD AT F.S. 531.0 LH SIDE WHERE IT ATTACHES TO THE FRAME, 13/16" IN LENGTH. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS I.A.W. EO 10-1900-53-048. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER. MAJOR REPAIR.
MAJOR REPAIR - DURING A SCHEDULED ROUNTINE INSPECTION A BULKHEAD AT FS 531 AFT CARGO FLOORBOARD FORWARD SUPPORT BUCKLED ABOUT 11/32" AT TOP, TOTAL DAMAGE AREA ABOUT 26" IN LENGTH, AND A .5" CRACK IN THE RADIUS 12" FROM RT SIDE. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 10-1900-53-047. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.
MAJOR REPAIR - DURING A SCHEDULED ROUNTINE INSPECTION A CRACK IN THE SKIN WAS FOUND ON THE EMERGENCY EXIT DOOR 6A, LOWER FORWARD CORNER, 2" IN LENGTH. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS I.A.W. EO 10-1900-52-003. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.
MAJOR REPAIR - DURING A SCHEDULED ROUNTINE INSP, A2 CRACKS WAS FOUND IN THE FRAME AT FS 456 AND STRINGER 10R CUTOUT, .45" AND .5" IN LENGTH. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 10-1900-53-046. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.
DURING A SCHEDULED ROUNTINE INSP A CRACK WAS FOUND IN THE FRAME ANGLE AT FS 303, .5" IN LENGTH. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS I.A.W. EO 10-1900-53-045. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.
COMPONENT MAJOR REPAIR - UPON RECEIVING A SERVICEABLE AIRSTAIR DOOR THAT WAS PURCHASED, IT WAS DETERMINED THAT IT REQUIRED A MAJOR REPAIR ON THE EXTERIOR DOOR SKIN DUE TO 3 SCRATCHES AND 1 PUNCTURE THROUGH THE SKIN. MX REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS IAW EO 10-1900-52-002. MATERIALS UTILIZED ARE INCLUDED IN THE EO.
PILOTS REPORTED THAT A STRONG FUEL ODOR FROM THE VENTS IN THE FLIGHT DECK AND CABIN WAS PRESENT. MAINTENANCE INSPECTED THE NR 1 AND 2 ENGINES FOR LEAKS OF WHICH NONE WERE FOUND AND SUBSEQUENTLY PERFORMED INDIVIDUAL ENGINE RUNS WITH NO FUEL ODORS NOTED. REFERENCE MM 71-10-00. AIRCRAFT WAS RETURNED TO SERVICE.
MAJOR REPAIR - AIRSTAIR DOOR WAS FOUND CORRODED ON LOWER END CLOSURE AREA. MAINTENANCE REMOVED A SECTION OF THE LOWER INNER DOOR PAN, CLEANED CORROSIONFROM INNER MATERIAL, INTALLED NEW LOWERE DOOR PAN AND INSTALLED THE HINGE ASSEMBLY IAW EO 09-1900-52-001.
ON APPROACH AT 500 LBS TORQUE, WHILE LEVELING ALTITUDE AND ADVANCING POWER THE R/H ENGINE ATTEMPTED FEATHER CAUSING CYCLES SIMILAR TO AUTOFEATHER TEST. MAINTENANCE REMOVED AND REPLACED THE RIGHT HAND AUTOFEATHER LOW PRESSURE SWITCH AND COMPLIED WITH FCD 1900-09-61-01 & FCD 1900-08-61-01. PERFORMED GROUND RUNS AND PERFORMED AUTOFEATHER TEST IAW MM 61-20-00. OPERATIONAL CHECK WAS GOOD.
SUSPECTED AUTO FEATHER - PILOTS REPORTED THAT ON SHORT FINAL THE RT ENGINE CYCLED IN AND OUT OF FEATHER. MX R&R THE A123 PRINTED CIRCUIT BOARD AND OPS CHECKED THE SYSTEM IAW MM 61-21-00.
SIGNIFICANT EVENT - PILOTS REPORTED THAT THE RT ENG AUTOFEATHERED IN FLIGHT AND CAUSED SURGES AND EXTREME YAWING MOTION. MX CLEANED AFX SWITCHES IAW BMM 61-21-01-201. OPS CHECKS WERE OK.
SMOKE/ODOR - PILOTS REPORTED THE GLARESHIELD INSTRUMENT EMERGENCY LIGHTS WERE SMOKING ON THE GROUND AFTER STARTING ENGINES. REMOVED EMERGENCY FUSE F127. MAINTENANCE REMOVED EMERGENCY FUSE F127 AND REMOVED AND REPLACED BULB IAW MM 33-10. OPERATIONAL CHECK GOOD.
FUSELAGE FRAME CRACKED APPROXIMATELY .8 INCH IN LENGTH AT FS 177 AT STRINGER 9AR. CRACKED AT STRINGER CUTOUT ON FRAME. REPAIRED FUSELAGE FRAME CRACK AT STRINGER RELIEF HOL AT F.S. 177 STRINGER 9AR. REPAIR APPROXIMATELY 4.4 INCH BY 2.5 INCH .080 2024 T-3.
FRAME CRACKED AT FS 164 AT STRINGER 9AL. CRACK LENGTH APPROXIMATELY 4 INCHES. 09-26-07 REPAIRED FRAME CRACK AT STRINGER RELIEF HOLE FS 164 AT STRINGER 9AR IAW BEECH 1900D SRM 51-70. REAPIR APPROXIMATELY 2.5" BY 4.1" .080 2024 T-3.
