CREW REPORTED ELECTRICAL FUMES IN FLIGHT. RIGHT VOR C/B WAS FOUND POPPED AND WOULD NOT RESET. RT VOR RECEIVER WAS FOUND TO HAVE STRONG ELECTRICAL SMELL AND SIGNS OF FAILURE.
LT SYS HYD QUANITY MSG FOUND LEAK IN LEFT WW. INSPECTED AND FOUND LT ELEC HYD PUMP HAS RUPTURED HOSE. REMOVED AND REPLACED RUPTURED HOSE. SERVICED AND LEAK CK IAW AMM12-12-01. NO LEAKS, NO DEFECTS NOTED.
A DENT AND GOUGE WERE FOUND IN THE SKIN AFT OF FS 1260 BELOW S-14R. THE DAMAGED SKIN IS WITHIN 3 INCHES OF THE SKIN SPLICE. THE DAMAGED AREA WAS CUTOUT AND A BOEING APPROVED REPAIR WAS INSTALLED PER 456588-14
CORROSION WAS FOUND ON THE NR 2 CARGO DOOR LOWER MAIN SILL OUTBOARD CHORD ON THE UPPER FACE OF THE OUTBOARD SURFACE OF THE HORIZONTAL FLANGE FROM APPROXIMATELY FS 1400 TO 1440. THE CORROSION WAS BLENDED PER SRM REQUIREMENTS WITH A MAXIMUM DEPTH OF 0.065", AND WAS APPROVED BY BOEING, AS DETAILED IN 456371-14.
CORROSION WAS FOUND ON THE FRAME ON THE INSIDE CHORD AT FS 729.6 ABOVE WL 210. THE DAMAGE WILL BE TRIMMED OUT AND BOEING APPROVED REPAIR ANGLE INSTALLED PER 456268-14.
LIGHTNING STRIKE DAMAGE AT A FASTENER LOCATION AND ADJACENT SKIN WAS FOUND AT FS 395 BETWEEN S-2L AND S-3L. THE DAMAGE WAS CUT OUT IAW MFG APPROVED INSTRUCTIONS, AND IS DOCUMENTED IN ERA 456266-14.
CORROSION WAS FOUND ON THE KEEL BEAM CHORD BETWEEN FS 660 AND FS 680, S-28R. THE CORROSION WAS REMOVED AND A BOEING APPROVED REPAIR INSTALLED PER 456557-14
A DENT WAS FOUND ON THE SKIN AT FS 223.50 BETWEEN WL200 AND WL 210, NEAR THE ANGLE OF ATTACH SENSOR. BOEING WAS CONTACTED AND AN APPROVED EXTERNAL REPAIR WAS INSTALLED PER 456555-14
ON INITIAL CLIMBOUT EXCESSIVE AIR PRESSURE FROM PACKS CAUSED A CEILING PANEL IN CABIN TO BLOW OFF WITH HIGH AIRFLOW ON PASSENGERS IN THAT AREA. REMOVED CEILING PANELS IN AFFECTED AREA, BLOWN OUT SECTION OF DISTRIBUTION DUCT AND NUMBER 5 VIDEO SCREEN REPAIRED. REPLACED 7 SECTIONS OF DISTRIBUTION DUCT ON N/R CARDS 1-11. REMOVED AND REINSTALLED CEILING PANELS AND VIDEO MONITORS ON N/R CARDS 1-11, RAN PACKS AND PRESSURE CONTROL SYS IN HIGH AND LOW FLOW W/ENGS AND APU CHECKS GOOD PER AMM.

THE LIGHTNING STRIKES WERE REMOVED WITH SMALL CUTOUTS. EACH CUTOUT WAS REPAIRED WITH AN EXTERNAL ALUMINUM REPAIR DOUBLER PER ER/A 385182-14, REV A.
LIGHTNING DAMAGE, CUTOUT REPAIR. THE FUSELAGE SKIN WAS STRUCK BY LIGHTNING AT FS 1450 AND STRINGER 6R. THE DAMAGE WAS REMOVED WITH A 1.2 INCH CUTOUT IN THE CROWN SKIN. THE CUTOUT WAS REPAIRED WITH 2 EXTERNAL ALUMINUM REPAIR DOUBLERS IAW ER/A 385187-14, REV A.

THE EXISTING DAMAGE CUTOUT AND THE FASTENER HOLES FROM THE TEMP REPAIR WERE REMOVED WITH A 8 X 8 INCH CUTOUT. THE NEW CUTOUT WAS REPAIRED WITH AN EXTERNAL ALUMINUM REPAIR DOUBLER INSTALLED PER ER/A 378711-14, REV B.
WHILE ACCOMPLISHING A DOUBLER MODIFICATION TO THE VHF ANTENNA CUTOUT AT FS 1460, THE SKIN WAS DAMAGED IN MULTIPLE AREAS BETWEEN STRINGERS 28L & 28R. THE DAMAGE WAS REMOVED WITH A CUTOUT IN THE SKIN. THE CUTOUT WAS REPAIRED BY MODIFYING THE EXTERNAL ALUMINUM ANTENNA DOUBLER TO INCORPORATE THE NEW DAMAGE PER ER/A 378637-14, REV A.
THE FUSELAGE SKIN WAS DENTED AND GOUGED AT FS 620 AND STRINGERS 16L AND 17L. THE DAMAGE WAS REMOVED BY CUTTING OUT THE SKIN OVER A 14.5 X 14.5 INCH AREA. THE CUTOUT WAS REPAIRED WITH AN EXTERNAL ALUMINUM REPAIR DOUBLER INSTALLED PER ER/A 378785-14, REV B.

AN EXISTING TIME-LIMITED, BLIND FASTENER REPAIR DOUBLER WAS INSTALLED PREVIOUSLY TO REPAIR A SMALL CUTOUT DUE TO A LIGHTNING BURN AT FS 520 AND STRINGER 8R. THE TEMPORARY REPAIR WAS REMOVED AND ALL REPAIR FASTENERS AND DAMAGED CUTOUT REMOVED WITH A SINGLE 12.5 X 7 INCH CUTOUT. THE NEW CUTOUT WAS REPAIRED WITH AN EXTERNAL ALUMINUM DOUBLER AND SOLID RIVETS PER ER/A 378648-14, REV A.
THE FUSELAGE SKIN NEAR FS 560 BETWEEN STRINGERS 7R AND 8R WAS PREVIOUSLY STRUCK BY LIGHTNING AND REPAIRED WITH AN EXTERNAL ALUMINUM DOUBLER ON A TIME LIMITED BASIS. THE TEMP REPAIR WAS REMOVED, 7.5 X 12 INCH CUTOUT. THE CUTOUT WAS REPAIRED WITH A NEW EXTERNAL ALUMINUM PERMANENT REPAIR DOUBLER PER ER/A 378647-14, REV A.
ELECTRICAL SMELL IN COCKPIT FOLLOWED BY SMOKE COMING FROM FWD GLARESHIELD. PWR REMOVED FROM AIRCRAFT WHICH CAUSED SMOKE TO DISSIPATE. FOUND BRAKE SOURCE LIGHT ASSY BURNED. REPLACED WIRE CONNECTORS AND LIGHT ASSY. NO DAMAGE TO SURROUNDING ASSYS. LIGHT OPS CKS NORMAL. MRR PARTS SENT AND PARTS ROUTED TO ATL DEPT 595 FAA LAISON.

EMERGENCY TRACK LIGHT MISSING AT SEAT 3B. REPLACED LIGHT AND LENS.
DURING S/C FOUND LEFT OVERWING SLIDE WOULD NOT TEST. FOUND LEFT BOTTLE EMPTY. REPLACED SAME PER MM 25-65-02, SYS TEST OK.
ON CLIMBOUT NOTICED EXCESSIVE FUEL CONSUMPTION ON LEFT ENGINE. FUEL IMBALANCE BECAME EXCESSIVE AND UNCONTROLLABLE, SHUTDOWN LEFT ENGINE AND FUEL CONSUMPTION BECAME NORMAL. FOUND LEFT ENGINE FUEL PUMP MAIN FILTER HOUSING CAP AND FILTER HAD BEEN BLOWN OFF. REPLACED FUEL PUMP AND FUEL CONTROL, OPERATION NORMAL.
RADOME WAS REMOVED FOR MOISTURE. SHOP INSPECTION FOUND OLD REPAIRS TO BE OF WET-LAY REPAIRS. CUT OLD SHELL OFF FROM BASE RING AND REBUILD RADOME COMPLETE PER SRM 53-10-72 USING 757 RADOME MOLD AND BOEING SPEC MATERIAL.
GROUND DAMAGE TO NOSE COWL AT 10 O'CLOCK POSITION. REPAIRED PER SRM 54-11-01.
GROUND DAMAGE TO NOSE LIP AT THE 11 O'CLOCK POSITION. REPAIRED PER SRM 54-11-01, FOR NR 2 INTAKE COWL AT SHOP INSPECTION.
GROUND DAMAGE TO NOSE COWL PANEL WAS FOUND AT 10 O'CLOCK POSITION, DURING HMV. DAMAGE WAS REPAIRED PER SRM 54-11-01 REF 51-70-17.
DURING SHOP INSPECTION, DELAMINATION WAS FOUND ON THE T/E WEDGE OF A NR 9 SLAT. THE WEDGE WAS REPLACED PER SRM 54-43-02.
ON INITIAL SHOP INSPECTION, MAINTENANCE FOUND 2 SMALL HOLES APPROX 1 INCH IN DIAMETER ON BOTTOM SKIN, LOCATED 4 INCHES FROM T/E. REPAIR WAS ACCOMPLISHED PER SRM 57-70-01 FOR NR 6 SPOILER PANEL.
FOUND DURING LETTER CHECK, RADOME HAS 2 PUNCTURES ON LT SIDE. REPAIRED PER SRM 53-10-72.
FOUND DURING LETTER CHECK, 2 LIGHTNING STRIKES 1 PREVIOUSLY PAINTED OVER UPPER FUS CROWN AREA 6 FEET FWD VERT STAB. REPAIRED PER SRM 53-00-01. TIME LIMITED REP MUST BE REINSPECTED PRIOR TO 2,500 CYCLES AND REPLACED PRIOR TO 12,OOO CYCLES, E/C INSPD OK AT THIS TIME.
