DEPARTING KBOI, DURING CLIMB, THE CREW TURNED ON ALL ANTI-ICE SYSTEMS (LEFT AND RIGHT WING AND ENGINE ANTI-ICE; TAIL DE-ICE (AUTO); WIND SHIELD ANTI-ICE (BLEED SWITCH LOW, LEFT AND RIGHT BLEED AIR KNOBS MAX). SHORTLY AFTER ENGAGING THE ICE PROTECTION SYSTEMS, THE RIGHT ENGINE ITT INDICATOR ON THE ENGINE INDICATION SYSTEM WAS DISPLAYING AN OVERTEMPERATURE. CREW ALSO OBSERVED AN AMBER WING A/I OVERTEMP R AND A T2 HEATER FAIL R CAS MESSAGE ON THE PFD. CREW NOTIFIED ATC AND REQUESTED RETURN TO KBOI. THEY DID NOT DECLARE AN EMERGENCY. THE LANDING BACK AT KBOI WAS UNEVENTFUL. MX HAS DOWNLOADED FADEC AND AIRFRAME DATA LOGS AND SENT TO TEXTRON AND WILLIAMS FOR EVAL. WE BELIEVE THE RIGHT ANTI-ICE VALVE FAILED AND ALLOWED FULL UN-REGULATED BLEED AIR TO BE SENT TO THE RIGHT WING. THIS OVER-TEMPED THE ENGINE AS WELL AS THE WING LEADING EDGE. WE HAVE PHYSICAL HEAT DAMAGE TO THE RIGHT WING LEADING EDGE IN THE FORM OF BUCKLING/CREASING. THE AIRCRAFT IS GROUNDED WAITING ON RESOURCES TO TROUBLESHOOT.
ON FLIGHT FROM KOSU TO KAVL, DURING DESCENT, FELT A SUDDEN VIBRATION FOLLOWED BY THE AIRCRAFT YAWING TO THE LEFT AND A SLIGHT OSCILLATION UP AND DOWN. THERE WAS A RED GEAR UNSAFE LIGHT ILLUMINATED AND A HYDRAULIC PRESSURE CAS MESSAGE WITH THE GEAR HANDLE STILL IN THE UP POSITION. SLOWED THE AIRCRAFT TO A SAFE GEAR DOWN OPERATING SPEED. LOWERED THE GEAR, RECEIVED 3 GREEN DOWN AND LOCKED AND NO RED LIGHT ON GEAR INDICATION. PROCEEDED WITH A NORMAL LANDING AT KAVL WITH NO INCIDENT. TEXTRON AOG TECH FOUND THE L MAIN GEAR UPLOCK HOOK CRACKED IN HALF.
DURING TAKEOFF, THE CREW REPORTED THE RH GEAR DOWN/LOCKED (GREEN) LIGHT DID NOT EXTINGUISH AND THE TRANSIT LIGHT STAYED ON. THE CREW FOLLOWED THE AFM PROCEDURE AND RESET AND ATTEMPTED A SECOND GEAR RETRACTION WITH NO SUCCESS. MX PERFORMED MULTIPLE GEAR SWINGS AND COULD NOT DUPLICATE THE DISCREPANCY. THE EVENT WAS DISCUSSED WITH TEXTRON TECH SUPPORT AND AT THEIR RECOMMENDATION, WE REPLACED THE RH GEAR REGEN SHUTTLE VALVE. OPS CHECK AND FLIGHT EVALUATION WERE SATISFACTORY.
THE CO-PILOT RIGHT, FORWARD SIDE WINDOW OUTER PANE SHATTERED DURING CLIMB THROUGH FL380 OUT OF ORLANDO, FL. CREW DIVERTED TO TALLAHASSEE AND TEXTRON'S MSU WAS CALLED TO REPLACE WINDOW. NEW WINDOW FAILED HEAT OPERATIONAL CHECK AND A BAD HEAT CONTROLLER WAS FOUND. HEAT CONTROLLER WAS REPLACED ON SITE AND OPERATIONALLY CHECKED SATISFACTORY.
CREW REPORTED DASHED LINES ON THE OIL PRESSURE INDICATION FOR THE LEFT ENGINE.
ACFT WAS UNDERGOING A 100 HR INSPECTION. REMOVED LT & RT MLG WHEEL ASSEMBLIES TO COMPLETE NDT AND UPON WHEEL REMOVAL ONE OF THE WHEEL BRAKE DISC ASSEMBLIES FELL ONTO FLOOR IN 2 SEPARATE PIECES. THE LT AND RT BRAKE ASSEMBLIES BOTH SHOWED SEPARATED BRAKE DISCS. THE BRAKE DISC ASSEMBLIES WERE NOT THE ORIGINAL BRAKE ASSEMBLIES. THE AFFECTED BRAKE ASSEMBLIES WERE INSTALLED AT AN EARLY DATE AND TAGGED AS REMOVED FROM ANOTHER ACFT. THE OPERATOR DID NOT NOTICE ANY ABNORMAL SOUNDS OR CONDITIONS DURING ACFT OPERATIONS. THE TECH DID NOT NOTICE ANY ABNORMAL SOUNDS OR CONDITIONS DURING RUN UP OPS AND THE WEAR GUIDE PINS SHOWED NORMAL. SYS FUNCTIONED PROPERLY.
DURING CRUISE FLIGHT THE PIC NOTED A HOT ELECTRICAL ODOR AND MINOR SMOKE IN THE RT SIDE COCKPIT AREA. THE PIC EXECUTED AN EMERGENCY DECENT AND DEPRESSURIZED THE CABIN FOR VENTILATION. THE FLIGHT WAS DIVERTED TO AN ALTERNATE (CLOSER) AIRPORT AND LANDED UNEVENTFULLY. IT WAS DISCOVERED BY MX THAT THE RT STALL HEAT CONNECTOR (E325D) IN THE COCKPIT OVERHEAD SWITCH PANEL HAD MELTED. IT WAS SUSPECTED THAT POOR CONTACT BETWEEN A PIN AND SOCKET AND THE HIGH AMPERAGE DRAW OF THAT SYS CAUSED EXCESSIVE HEAT BUILD UP IN THE CONNECTOR. THE CONNECTOR WAS REMOVED, THE WIRES TRIMMED BACK TO GOOD WIRE AND WERE SPLICED TOGETHER, BYPASSING THE CONNECTOR. OPERATION OF AFFECTED SYS WAS CHECKED OK. THE ACFT RETURNED TO HOME BASE.
ON JUNE 27, 2007 AT APPROXIMATELY 6:25 PM, WHILE ON A MEDICAL FLIGHT, WAS CLIMBING THRU 20,000 FT FOR 22,000 FT, THE LT NR 2 CABIN SIDE WINDOW SUFFERED AN UNFORESEEN CATASTROPHIC FAILURE. AN EMERGENCY WAS DECLARED TO AIR TRAFFIC CONTROL AND THE AIRCRAFT LANDED SAFELY WITH NO FURTHER INCIDENT. THE INCIDENT WAS REPORTED IMMEDIATELY TO THE FAA. THE BROKEN WINDOW WAS REPLACED WITH APPROVED REPLACEMENT WINDOW. THE AIRCRAFT PASSED ALL SUBSEQUENT POST MAINTENANCE AND FLIGHT TESTS AND WAS RETURNED TO SERVICE. THE BROKEN WINDOW WAS GIVEN TO THE FAA. (K)
