AFTER TAKE OFF NOSE WHEEL WAS 30 DEGREES TO THE LEFT NOT STRAIGHT.
SMOKE IN CABIN AFTER EMERGENCY PRESSURIZATION VALVE OPENED, CREW ON A 91 FLIGHT LANDED AS SOON AS POSSIBLE IN GJT.
WHILE IN CRUISE AT FL380, THE LT GEN ANNUNCIATOR ILLUMINATED. AT THE SAME TIME, AUTOPILOT DISCONNECTED AND BELL/ANNUNCIATOR ILLUMINATED. FOLLOWED BY SIGNIFICANT ELECTRICAL SMOKE AND FUMES FROM BEHIND THE INSTRUMENT PANEL. AFTER TROUBLESHOOTING, FOUND THE CABIN ACFT ELECTRICAL OUTLET INVERTER LOCATED UNDERNEATH THE COPILOT'S SEAT HAD SHORTED INTERNALLY CAUSING THE SMOKE.
WHILE EN ROUTE TO CEDAR RAPIDS, IOWA FROM JVY RIGHT ENGINE FAILED DURING CLIMB AT ABOUT 30,000 FEET. TURBINE RPM DROPPED TO ZERO. LANDING MADE AT IND WITHOUT INCIDENT. UPON INITIAL INSPECTION NOTED FANS SPINS FREELY BUT TURBINE WILL NOT ROTATE. (M)
WHILE ENROUTE RIGHT ENGINE FAILED DURING CLIMB AT ABOUT 30,000 FT. TURBINE RPM DROPPED TO ZERO. LANDING MADE WITHOUT INCIDENT. UPON INITIAL INSPECTION NOTED FAN SPINS FREELY BUT TURBINE WILL NOT ROTATE. ENGINE WAS REMOVED AND SENT FOR REPAIR OF NR2 SPOOL LOCKUP. TEARDOWN REVEALED ENGINE HAD SUFFERED NR5 BEARING FAILURE.
SDL/IND - WHILE EN ROUTE AFTER CLIMB-OUT TO 40,000 FEET MSL, FLIGHT CREW NOTICED CABIN PRESSURIZATION CLIMBING. FLIGHT CREW MADE A PRECAUTIONARY EMERGENCY DESCENT AND DIVERTED TO GUP. MAINTENANCE REMOVED AND REPLACED BLEED AIR REGULATING VALVES. AIRCRAFT RETURNED TO SERIVCE.
