MOMENTARY LOSS OF AIRSPEED AND ALTIMETER. HAPPENED 2 TIMES IN FLIGHT.
RT STAB ANTI ICE BOOT. ANTI ICE BOOT CAUGHT ON FIRE AND BURNED A HOLE AFTER MAIN BAT 1 & 2 WERE SWITCHED ON
AHS 1 & 2 FAILURE. HAPPENED IN CRUISE, AHS 1 & 2 ANNUNCIATORS. CAME BACK TO LIFE AFTER WE WERE ALREADY HEADING BACK TO BASE. WHILE TROUBLESHOOTING THIS ISSUE #2 SYMBOL GENERATOR FAILED PN 066-04021-1111 OFF SN 4932, ON SN 2635. BOMBARDIER TECHNICAL SUPPORT RECOMMENDED REPLACING THE EFS SWITCHING UNIT PN 071-00074-0500 OFF SN 1480, ON SN 1291.
RIGHT ENGINE FIRE WARNING TEE HANDLE LIGHT CAME ON IN FLIGHT. REMOVED RT FIRE DETECTION CONTROL UNIT P/N 37702833 S/N 0143-069 AND INSTALLED REMOVED SERVICEABLE FIRE DETECTION CONTROL UNIT P/N 377-02833, S/N 9938-006 IAWL31A 26-10-01. OPERATIONAL CHECK GOOD AT AT THIS TIME. REMOVED RIGHT ENGINE T/R ASSY., UPPER &LOWER INNER FAN DUCT PANELS. INSTALLED INSPECTED/ TESTED HOT SECTION SENSING ELEMENT PN 244-07235 REMOVED FROM ENGINE SN P-99515 IAW LEARJET 31 SERIES M.M. 26-10-02. INSTALLED UPPER & LOWER INNER FAN DUCT PANELS AND T/R ASSY. IAW TR4000 M.M. 78-31-00. PERFORMED ENGINE RUN AND T/R TEST. OPS &LEAK CHECK OK.
FLAP INDICATION WAS NOT INDICATING CORRECTLY IN ALL POSITIONS.
DURING FLIGHT A SMALL AMOUNT OF SMOKE CAME FROM CENTER PEDESTAL WHEN INTEG LIGHTS WERE TURNED ON. WHEN INTEG SWITCH WAS TURNED OFF SMOKE DISSIPATED. NO CIRCUIT BREAKERS POPPED.
AT FLIGHT LEVEL 410 AIRCRAFT HAD UN-COMMANDED SLOW PITCH DOWN AT 15 DEGREES. PILOT INITIATED PROPER EMERGENCY PROCEDURE AND THEN MADE PRECAUTIONARY LANDING.
AIRCRAFT BRAKES FAILED ON GROUND WHILE TAXING WITH ANTI-SKID ACTIVATED, DEACTIVATED ANTI-SKID SYSTEM BRAKES WORKED NORMALLY AS THEY WOULD WITHOUT ANTI-SKID. FERRIED AIRCRAFT TO HOME BASE AND FURTHER TROUBLESHOT SYSTEM AND FOUND RIGHT MAIN GEAR SAFETY SWITCH DEFECTIVE. REPLACED SWITCH WITH NEW AND OPS CHECKED OK.
DURING ROUTINE INSPECTIONS, THE RIGHT HAND INBOARD GEAR DOOR INNER SKIN IS FOUND CRACKED NEAR THE BOTTOM BOLT HOLE WHERE THE INBOARD GEAR DOOR ACTUATOR ATTACH FITTING MOUNTS TO THE DOOR. THE CRACK APPEARS TO BEGIN UNDER THE ATTACH FITTING, AND RADIATES OUTWARD IN BOTH FORE AND AFT DIRECTIONS. THERE IS OFTEN A LATERAL COMPONENT TO THE PROPAGATION. THE CRACK IS NOT VISIBLE UNTIL IT EXTENDS PAST THE FITTING, THIS IS A CRACK LENGTH IN EXCESS OF ABOUT 0.75 INCH. THIS CONDITION HAS BEEN OBSERVED MULTIPLE TIMES, WITH TOTAL CRACK LENGTHS UP TO 4 INCHES.
OIL DIP STICK O-RING FAILED, REPLACED WITH NEW AND SERVICED ENGINE OIL.
STARTER GENERATOR FAILED DURING FLIGHT, CREW NOTICED VIBRATION FOLLOWED BY ILLUMINATION OF THE LEFT GENERATOR LIGHT. AIRCRAFT LANDED WITHOUT INCIDENCE AND STARTER GENERATOR WAS REPLACED.
STARTER GENERATOR FAILED DURING FLIGHT, CREW NOTICED VIBRATION FOLLOWED BY ILLUMINATION OF THE LEFT GENERATOR LIGHT. AIRCRAFT LANDED WITHOUT INCIDENCE AND STARTER GENERATOR WAS REPLACED.
RIGHT ENGINE CHIP DETECTOR LIGHT ILLUMINATED DURING FLIGHT. AIRCRAFT WAS EN ROUTE TO PICK UP PASSENGERS. AIRCRAFT RETURNED TO HOME BASE. DOM REMOVED CHIP DETECTOR AND FOUND TINY SLIVERS OF METAL ACROSS CONTACTS. SPECIAL OIL SAMPLE TAKEN AND SENT TO LAB. OPERATIONAL CHECK OF CHIP DETECTOR COMPLETED. ENGINE RUN-UP AND LEAK CHECK FOUND OK. WILL RESMAPLE IN 10 FLIGHT HOURS.
