ON FRIDAY, AUGUST 21ST AT APPROX 1900Z, ACFT DEPARTED EMPTY & ENTERED CRUISE, EVERYTHING WAS NORMAL. ON THE INITIAL DESCENT CREW HEARD A STRANGE NOISE & OBSERVED AN ENG COMPUTER LIGHT FOR THE NR 2 ENG. ALL OF THE INSTRUMENTS FOR THE NR 2 ENG THEN FLUCTUATED & FELL TO ZERO. SHUT THE ENG DOWN & SECURED IT VIA THE CHECKLIST. CREW ADVISED AT NO POINT WAS THE ACFT OUT OF CONTROL. THE DESCENT & APPROACH WAS STABLE AT ALL TIMES. LANDED SAFELY & WITHOUT INCIDENT.

DURING A POSTFLIGHT INSPECTION, DISCOVERED THE RT AIRBRAKE OTBD COVERS WERE NOT ALIGNED. FOUND BOLTS IN BRACKET WERE LOOSE. SAFETY WIRE WAS STILL INTACT. SAFETY WIRE REMOVED AND AN ATTEMPT WAS MADE TO TORQUE THE BOLTS, NO JOY. BOLTS REMOVED AND FOUND SEVERELY CORRODED AND ERODED TO WHERE THEY WERE NOT SECURING THE ATTACH BRACKET.

Part: ACCESSORY UNIT — DAMAGED (STARTER)

DURING A 75 HOURS SOAP SAMPLE AFTER MPI A WASHER WAS FOUND ON THE CHIP DETECTOR ON THE ACCESSORY GEAR CASE. IT WAS IDENTIFIED AS A WASHER FROM THE STARTER FAILURE THAT THE SHAFT CAUSED THE HOLDING PAD MOUNT TO BRAKE. THE LAST STARTER FAILURE WAS IN 2005. FROM MFG THE TEARDOWN REPORT INDICATED THE SHAFT WAS DAMAGED. MFG AT THIS POINT DOESN`T HAVE A PROCEDURE FOR INSPECTION OF THE ACCESSORY GEARBOX AFTER STARTER FAILURE. RECOMMENDATION IS TO PERFORM INSP OF CHIP DETECTOR AFTER STARTER FAILURE. DUE TO FINDINGS OF THE WASHER THE ACCESSORY GEAR CASE WAS REMOVED FROM SERVICE AND REPLACED WITH O/H UNIT. THIS WAS RECOMMENDED BY MFG SERVICE TECH. (K)

Part: PUMP — FAILED (NR 1 ENG FUEL)

ON THE EVENING OF FRIDAY, JULY 18TH, ON CLIMB, EN ROUTE, THE CREW OBSERVED THE LT ENGINE, SN 91535, FUEL LIGHT ILLUMINATED. THE CREW REDUCED POWER. ENGINE THEN EXPERIENCED AN UNCOMMANDED INFLIGHT SHUTDOWN. CREW SECURED THE ENGINE AND ATTEMPTED A RESTART. RESTART WAS UNSUCCESSFUL. CREW THEN EXECUTED AN AIR TURNBACK AND SIGNALED AN EMERGENCY LANDING. TROUBLESHOOTING HAS REVEALED THAT THE FUEL PUMP FAILED A FUNCTIONAL CHECK. (K)
