the collapse of the left main landing gear during landing roll as result of a shorted weight-on-wheels relay. Contributing factors were the insufficient information available to the flight crew in the company's quick reference handbook (QRH), and the manufacturer's inadequate protection of the weight-on-wheels relay from moisture.

the engine fire during descent resulting from a loose, leaking fuel transfer tube due to improper installation and alignment of the transfer tube's retaining clip. Contributing factors were the inadequate design of the clamp, the failure of the operator to comply with the recommendations of the manufacturer's Service Bulletin (SB), and the failure of the FAA to issue an Airworthiness Directive to mandate the manufacturer's SB.

Total loss of power in the left engine from ice ingestion; and failure of the left-engine auto-relight system caused by an open fuse in the Digital Electronic Control Unit (DECU), which prevented an automatic relight of the left engine. Factors relating to the incident were: inadequate design of the engine air inlet anti-ice system, which allowed ice/slush accumulation; inadequate action by the FAA to ensure compliance with certification requirements regarding inlet ice accumulation; and an inadequate procedure in the flight manual concerning use of the engine synchrophaser during single-engine operation.

improper installation of the #7 and #8 fuel nozzle manifold transfer tubes by company maintenance personnel, and an inadequate exhaust drain fitting gasket, which resulted in fuel leaks at the respective 'O' ring/gasket fittings; blockage of the fuel/oil drain manifold; and subsequent fire in the engine compartment.

the flight attendant's failure to follow procedures which resulted in her inadvertently opening the main cabin door in flight. Factors associated with the incident were: the flight attendant's fatigue due to lack of sleep and a long duty day, interference with habit regarding the direction of motion of the door locking handle, and inadequate design of the door locking handle by the manufacturer.
