The failure of the crewmembers of the second helicopter to maintain visual separation with the first helicopter while complying with the LC controller’s instruction to go around. Contributing to the accident were the nighttime conditions and the lack of a safety alert from the controller to either helicopter.

The dual student's improper use of the collective and the flight instructor's inadequate supervision during the practice hovering autorotation maneuver. A factor was the underlying soft dirt terrain, which promoted the rollover event.

the pilot's intentional VFR flight into IMC conditions and failure to maintain an adequate terrain clearance altitude, and the local controller's improper use of ATC procedure/technique. Factors in the accident were the helicopter's low fuel state and lack of the required equipment for IFR flight.

LEFT FORWARD SEAT BELT BUCKLE WOULD NOT RELEASE AFTER LANDING. SEAT BELT HAD TO BE DISCONNECTED FROM AIRCRAFT TO ALLOW INSTRUCTOR PILOT TO EXIT. POTENTIALLY DEADLY IF THERE HAD BEEN AN EMERGENCY LANDING.

BEVEL REDUCTION MODULE (PN 350A32-0300-05, SN M1899) WAS REMOVED AT TSO: 2367.5 FOR METAL CONTAMINATION. UPON TEARDOWN ANALYSIS IT WAS DISCOVERED THAT THE BEARING CAGE FOR BEARING (P/N 704A33-652-036) TSN: 2367.5, WAS BROKEN. SUBSEQUENT DETERIORATION OF BEARING WAS CAUSE FOR METAL CONTAMINATION.
