Part: DOOR FRAME — CRACKED (BS 243.3)
DURING INSPECTION, A CRACK WAS FOUND ON THE LOWER LEFT PORTION OF THE CABIN DOOR FRAME AT BS 243.38. THE CRACK ORIGINATES FROM A BEND RELIEF CUTOUT IN A HEAVILY FORMED SECTION OF THE FRAME. THIS IS THE THIRD AIRCRAFT WE FOUND THIS CRACK PRESENT, SAME MAKE AND MODEL. THE REGION OF THE FRAME HAS TO BE DISASSEMBLED AND INSPECTED FOR OTHER SIGNS OF DAMAGE OR DISTRESS. AN ENGINEERED DOUBLER HAS TO BE INSTALLED WITH STRUCTURAL ADHESIVE AND RIVETS.
Part: FASTENER — BROKEN (CABIN FLOOR)
THE ADJUSTABLE GRIP LATCH FASTENER THAT HOLDS DOWN THE CENTER ISLE FLOORBOARD IN THE CABIN FALLS APART AND CONDUCTIVE PIECES FALL INTO THE ELECTRICAL EQUIPMENT BELOW THE FLOORBOARDS. A PIECE WAS FOUND ON THE FLAP RELAY DURING REGULAR MAINTENANCE.
Part: STABILIZER — RIVETS POPPED (V STAB ATTACH)
INSPECTION OF RUDDER AND VERTICAL STABILZIER AREA REVEALED RIVETS WITH POPPED HEADS ON ATTACHMENT FITTING FOR AFT SPAR ON VERTICAL STABILIZER-TO-EMPENNAGE ATTACH POINT. (SIX OUT OF EIGHT RIVETS WERE SHEARED). THOROUGH INSPECTION OF EMPENNAGE REVEALED NO OTHER PROBLEMS.

THE FLAP LEVER ATTACH BRACKET IS CRACKED AT THE FORWARD LOWER BEND RADIUS, ADJACENT TO THE ATTACH POINT TO THE FLOOR.

DURING A MAGNETO 500-HOUR INSPECTION, ONE MOUNTING HOLE FOR THE DISTRIBUTER BLOCK WAS FOUND CRACKED.

DURING MAGNETO 500-HOUR INSPECTION, ONE MOUNTING HOLE OF THE DISTRIBUTOR BLOCK WAS FOUND CRACKED.
Part: RIVET — BROKEN (VERT STAB)
THE SHOP HEAD OF THE RIVET ON THE RIGHT SIDE OF THE VERTICAL STABILIZER, JUST ABOVE THE FUSELAGE THAT HOLDS THE AFT INNER CHANNEL TO THE SKIN, WAS BROKEN OFF. THIS MAY HAVE BEEN CAUSED BY VIBRATION. SUBMITTER KNOWS OF SEVERAL INSTANCES OF SAME RIVETS ON BOTH SIDES BREAKING ON FLEET AIRCRAFT.
