CREW RECEIVED A HYRAULIC CAS MESSAGE WHILE IN CURISE. AIRCRAFT DIVERTED TO NEAREST AIRPORT. INSPECTION BY MAINTENANCE PESONNEL REVEALED A HYDRAULIC LINE TO HAEVE AT A PERASWAGED T-FITTING LOCATED ABOVE FWD RH SIDE OF MLG TORQUE BOX, RT. WING.
CREW SMELLED SMOKE IN COCKPIT, DECLARED AN EMERGENCY. ON DECENT CREW NOTICED THE SMOKE IN THE VICINITY OF THE PILOT'S WINDSHIELD. CREW CYCLED OF THE WINDSHIELD HEAT AND THE SMOKE DISSIPATED. ONCE ON THE GROUND, MAINTENANCE TROUBLESHOOTING CONFIRMED THE WINDSHIELD CAUSED THE ISSUE. THE WINDSHIELD WAS REPLACED. A COMPANY HAZARD REPORT WAS FILED, AND THE DISCREPANCY WAS LOGGED IN THE COMPANY CASS SYSTEM.
DURING FLIGHT PILOT SIDE OUTER PANE CRACKED
DISCRPANCY:PILOT NOTIFIED MAINTENANCE AND OPERATIONS IN FLIGHT THE #1 & # 2 FLIGHT GUIDANCE COMPUTER IS INTERMITTENT, AND THE PITCH TRIM DISENGAGED. CORRECTIVE ACTION:GULFSTREAM MAINTENANCE FOUND A PITCH TRIM SERVO DRUM AT FAULT. THE PITCH TRIM SERVO DRUM AND CABLES WERE REPLACED, AND OPERATIONAL CHECKS WERE COMPLETED. THE DEFERRED
AIRCRAFT DEPARTED SOUDA BAY IN ROUTE TO BANGKOK WHEN THE RIGHT ENGINE UNCOMANDED SHUTDOWN. THE CREW FOLOWED THE AFM PROCEDURES AND ATTEMPTED AN AIR START AND WAS UNSUCCESSFUL.THE AIRCRAFT LANDED AT THE INTENDED AIRPORT WITHOUT INCIDENT. THE ENGINE WAS INSPECTED ON WING AND WAS DETERMINED REJECTED BY THE ENGINE MANUFACTURER.
THE AIRCRAFT CAME IN FOR AN INSPECTION AND WE NOTICED THE BOTTOM ROW OF RIVETS NEXT TO THE FUSELAGE ON BOTH SIDES OF THE VERTICAL STABILIZER HAVE BEEN WORKING. WE CONTACTED GULFSTREAM AND ARE WORKING TO GET A FIX FOR THE ISSUE.
MAINTENANCE FOUND IN THROTTLE QUADRANT ON RT SIDE IMPROPER PART (YELLOW PLASTIC SHIM) FOR SWITCH BLOCKING RT LOCKOUT PAWL NOT ALLOWING LOCKOUT SOLENOID TO PULL PROPERLY. INSTALLED NEW SWITCH AND BEGAN CHECK OF T/RS. RT T/R DEPLOYED ON ITS OWN. TROUBLESHOT AND FOUND NEW SWITCH DEFECTIVE. REMOVED DEFECTIVE NEW SWITCH AND INSTALLED ANOTHER SWITCH, PN 3115M7-T AND NEW SHIMS, PN 2615172-12 IN QUADRANT.
TIRE, TREAD SEPARATION ON TIRE IS OCCURRING WITH ALARMING FREQUENCY. 102 LANDINGS APPEAR TO BE SLIGHTLY MORE THAN .5000 OF ITS EXPECTED LIFE.
