DURING TAKEOFF THE PILOT FELT A HEAVY VIBRATION, THE KAPA TOWER WAS NOTIFIED, AN EMERGENCY DECLARED, A FLY OVER DEEMED THE TIRE STILL ATTACHED AND INFLATED. THE AIRCRAFT LANDED WITHOUT INCIDENT AND TAXIED TO MAINTENANCE. UPON INSPECTION THE TREAD OF THE LEFT MAIN TIRE WAS MISSING BUT PROPERLY INFLATED. THE AIRCRAFT SUSTAINED MINOR DAMAGE TO THE LOWER FLAP. THE WHEEL AND TIRE WAS REPLACED WITH A REPAIRED/NEW UNIT AND DEPARTED UNDER A SPECIAL FLIGHT PERMIT FOR FLAP REPAIRS.
TOWER REPORTED VAPOR COMING FROM RIGHT WING O/B OF AILERON, STOPPED VENTING SECONDS LATER. NO REPORTED LOSS ON FUEL GAUGES. UPON ARRIVAL, THE FUEL VENT VALVE WAS FOUND TO BE TORN. VENT VALVE WAS REPLACED WITH NO FURTHER PROBLEMS.
AFT FUSELAGE RIB P/N 500-06193-001 WAS FOUND CRACKED THROUGH DURING A 120 MONTH INSPECTION ITEM Z320-002 TASK CODE 05-43-00-210-804-A/600.
THE AIRCRAFT WAS ON A MAINTENANCE CHECK FLIGHT DUE TO AN ENGINE CHANGE. AFTER TAKEOFF, THE GEAR WOULD NOT RETRACT. THE AIRCRAFT RETURNED, MAINTENANCE JACKED THE AIRCRAFT, PULLED UP THE FIM AND FOUND THE NOSE GEAR SAFETY PROX SWITCH TARGET LOOSE AND NOT ACTUATING THE PROX SWITCH. THE TARGET WAS SECURED, THE GEAR SWING WAS NORMAL AND THE AIRCRAFT TOOK OFF AND COMPLETED ITS MCF WITH NO FURTHER DIFFICULTIES.
AFTER TAKEOFF, DURING CLIMB OUT, CREW ENGAGED THE AUTOPILOT AND EXPERIENCED AN UNCOMMAND AUTOPILOT PITCH AND YAW. DISCONNECTED THE AUTOPILOT AND RETURNED TO DEPARTURE.
DURING CRUISE AT FL410, THE CABIN PRESSURE BEGAN CLIMBING OVER 2000 FPM. RAPIDLY RISING TO FL270. EXECUTED AN EMERGENCY DESCENT AND LEVELED OFF AT 17,000 FT AND RETURNED TO DEPARTURE.
HYDRAULIC NR 1 SYSTEM, 'HYD 1 LO QTY' CAS MESSAGE ON MFD. WITH VISUAL VERIFICATION OF HYDRAULIC FLUID AFTER LANDING.
ENROUTE, THE AIRCRAFT EXPERIENCED A CABIN ALTITUDE CLIMB UNABLE TO HOLD CABIN ALTITUDE. AIRCRAFT DESCENDED AND RETURNED TO DEPARTURE. FOUND THE A/C PACK UNSECURE.
JANUARY 9, AIRCRAFT WAS IN TEB AND UPON TAXI OUT, CREW FELT A PRESSURE BUMP AND REPORTED AN ASSOCIATED CAS MESSAGE OF PRESS AUTO FAIL. AIRCRAFT RETURNED TO THE FBO. A TECHNICIAN WAS DISPATCHED AND PERFORMED TROUBLESHOOTING PER FIM. NO FAULTS FOUND AND MESSAGE WAS NOT IN VIEW. PER THE FIM IT STATES THAT IT WAS TRANSITORY CONDITION. A/C WAS RELEASED. A/C TOOK OFF, CLIMBED TO FL 380 AND WAS THERE FOR 20 MINUTES AND CABIN PRESSURE BECAME UNCONTROLLABLE. A/C DESCENDED TO FL 100 AND DIVERTED TO MDW. IN MDW THE OUTFLOW VALVES WERE LOCKED OUT AND WE INSTALLED MEL 21-30-00C. JANUARY 10, THE A/C WAS DISPATCHED TO CLE WHERE QUALIFIED MAINTENANCE IS BUT DUE TO RUNWAY ICE, THE AIRCRAFT DIVERTED TO CMH. AN AOG TEAM FROM CLE WAS DISPATCHED TO CMH. THE TECHNICIAN DOWNLOADED THE CMC AND SENT TO TECH SUPPORT AND STARTED FOLLOWING THE FIM FOR THE STATED ISSUE. REPORTED THAT NO ASSOCIATED MESSAGES WERE LOCKED INTO THE CMC SYSTEM. TROUBLESHOOTING SHOWED 1 STEP TO BE OUT OF TOLERANCE REGARDING DELTA P WHEN AIRCRAFT CONFIGURED ON GROUND. IT WAS RECOMMENDED TO CHANGE THE ELECTROMECHANICAL OUTFLOW VALVE AND PRESSURE CONTROLLER. ABOVE PARTS ORDERED, RECEIVED, INSTALLED. PRESSURIZATION SYSTEM FUNCTIONAL CHECK PERFORMED. SYSTEM FUNCTIONAL CHECKS GOOD. JANUARY 12, THE AIRCRAFT WAS DISPATCHED CMH-TEB TO POSITION FOR A REVENUE FLIGHT. IN ROUTE AND AT FL 390 FOR 20 MINUTES, CABIN PRESSURE STARTED CLIMBING. CREW DESCENDED TO FL 100 AND DIVERTED TO CLE. CLE MAINTENANCE TEAM DOWNLOADED CMC AND SENT TO TECH SUPPORT, NO FAULTS NOTED. RAN SUGGESTED FIM AND FOUND PNEUMATIC OUTFLOW VALVE FAILED THE TIME TO CLOSE TEST. ORDERED AND INSTALL OF OUTFLOW VALVE AND PERFORMED GROUND FUNCTIONAL CHECKS GOOD. WE PERFORMED A LOCAL MAINTENANCE FUNCTIONAL CHECK FLIGHT AND THE AIRCRAFT AND PRESSURIZATION SYSTEM OPERATED NORMALLY.
DURING EMERGENCY LIGHT TEST, ROW 6-10 FAILED TO ILLUMINATE.
DURING MAIN ENTRANCE DOOR OPERATION, THE DOOR GOT HUNG UP AND WOULD NOT OPEN. ONCE IT WAS OPENED, IT WAS DISCOVERED THAT THE DOOR STEP ACTUATION SUPPORT WAS CRACKED. THE AREA WAS INSPECTED FOR FURTHER DAMAGE, NONE WAS FOUND. PART WAS REPLACED AND DOOR OPERATIONAL CHECKED GOOD. SB 145LEG-52-0007 RELATES TO THIS DISCREPANCY. THIS SB HAD NOT BEEN COMPLIED WITH ON THIS ACFT AND WILL BE SCHEDULED AT EARLIEST CONVENIENCE.
