ROUGHNESS FELT DURING FLIGHT COMING FROM THE ENG. NR 2 CYLINDER FAILED.
BENDIX ON STARTER OF RT ENGINE FAILED TO DISENGAGE AFTER STARTUP. R & R STARTER WITH NEW. NO FURTHER DISCREPANCIES NOTED.
PILOT SMELLED ODOR AND SAW SMOKE FROM INSTRUMENT PANEL. PILOT RETURNED TO KODIAK AIRPORT WITHOUT INCIDENT AND MADE A NORMAL LANDING. ON INVESTIGATION, IT WAS FOUND THE RIGHT ENGINE ALTERNATOR VOLTAGE REGULATOR HAD CAUSED AN OVERVOLTAGE SITUATION AND THE SMOKE WAS CAUSED BY THE BATTERY BEING OVERCHARGED. A NEW REGULATOR WAS INSTALLED. THE OVERVOLTAGE ALSO CAUSED SEVERAL ENGINE INDICATING INSTRUMENTS TO BE REPLACED. AFTER REPAIRS, THE AIRCRAFT WAS RETURNED TO SERVICE.
PILOT EXPERIENCED AN ROUGH RUNNING LT ENGINE 20 MINUTES INTO THE FLIGHT. THE PILOT REPORTED THAT ALL LT ENGINE INSTRUMENT INDICATIONS WERE NORMAL AT THAT TIME. OIL PRESSURE DROPPED TO ZERO, TURNED BACK TO AIRPORT. DECLARED AN EMERGENCY AND LANDED WITHOUT ANY FURTHER INCIDENT. AFTER INSPECTION OF THE LT ENGINE, A HOLE WAS FOUND IN THE TOP OF THE ENGINE CASE BETWEEN CYLINDERS 5 AND 6. AT THE TIME OF THIS REPORT, THE LT ENGINE HAD NOT BEEN TORN DOWN TO INVESTIGATE THE CAUSE FURTHER. IT APPEARS THAT A POSSIBLE CONNECTING ROD AND BEARING WOULD BE THE CAUSE OF THE FAILURE.
RT MAIN GEAR UNSAFE LIGHT ON WITH GEAR RETRACTED.
THE ADC INSTALLED AS PART OF THE EFIS SYSTEM IAW STC NR SA02207AK WOULD NOT MAINTAIN A CONSTANT ACCURATE ALTITUDE READINGS. EACH TIME THE READING WAS RESET, THE ALTITUDE READING WOULD DEVIATE UP TO 2000 FT.
DISPLAY HAS AN INTERMITTENT LINE ACROSS MIDDLE OF SCREEN.
ATTEMPTED TO INSTALL 5.0 SOFTWARE, SCREEN WENT BLANK HALFWAY THROUGH UPGRADE.
DURING GROUND FUNTIONAL TEST OF THE INITIAL INSTALLATION OF AIR DATA COMPUTER (ADC) INSTALLED AS PART OF THE CHELTON FLIGHT SYSTEMS EFIS-SV SYSTEM UNDER STC SA02203AK, ENCOUNTERED 100FT ALTITUDE DRIFT WITHIN 5 MINUTES AFTER BAROMETRIC PRESSURE HAD BEEN SET.
UNIT REPORTING ERRONEOUS ALTITUDE READINGS UP TO 1000 FEET OFF. (AL05200401238)
PILOT STATED THAT DURING CRUISE FLIGHT, PROPELLER WENT TO FINE PITCH, AND OPERATION OF GOVERNOR CONTROLS HAD NO EFFECT. UPON GROUND RUN UP IT WAS FOUND THAT PROP WOULD NOT CYCLE. REMOVED FROM SERVICE. UNKNOWN CAUSE FOR FAILURE AT THIS TIME. (AL05200401344)
