NR 6 CONECTING ROD FAILED IN FLIGHT.
RH ENGINE WARNING FIRE LIGHT ILLUMINATED 10 MINUTES BEFORE LANDING IN DESTINATION, STAYED ON FOR 5 MINUTES.
RETRACTED GEAR AFTER TAKE OFF, HANDLE RETURNED TO NEUTRAL, NOSE GEAR SLOWLY PARTIALLY EXTENDED. CONTINUED TO DESTINATION GEAR EXTENDED NORMALLY.
1193 TSO ALUMINUM SLEEVE INSTALLED AT UNKNOWN TIME SEIZED ON MAGNETO DRIVE GEAR AND WAS SPINNING IN CASE BORE.
PILOT RH BRAKE FAILED WHILE TAXIING. FOUND 1/4" ALUMINUM LINE BELOW FLOOR CORRODED PIN HOLE.
NOSE GEAR WARNING LIGHT DID NOT ILLUMINATE WHEN EXTENDED.
NOSE GEAR WARNING LIGHT DID NOT ILLUMINATE WHEN EXTENDED. LIGHT CAME ON AT TOUCH DOWN. LUBED GEAR AND AJUSTED NOSE ACTUATOR. TEST GOOD. AMENDMENT TO 2022F00098 02/23/2022 ONLINE
PILOT'S DOOR POPPED OPEN IN FLIGHT, PILOT DIVERTED TO THE NEAREST AIRPORT, LANDED AND RE-LATCHED THE DOOR. THEN CONTINUED ON TO DESTINATION. ONCE THERE, THE UPPER DOOR LATCH CHANNEL (AIRFRAME SIDE) WAS FOUND TO BE LOOSE AND WAS TIGHTENED AND RETURNED TO SERVICE.
DURING CRUISE POWER SETTINGS ALTERNATOR BELT WOULD BE THROWN OFF. AT RECOMMENDATION OF ENGINE SHOP, REMOVED CYLINDERS ONE AND TWO. REMOVED COUNTERWEIGHT AND FOUND PINS AND BUSHINGS TO BE WORN BEYOND LIMITS LIKELY CAUSING VIBRATION.
FIBERGLASS TORE ALONG THE LEADING EDGE FASTENERS OF THE COWLING AND THE UPPER COWLING BEGAN TO PEEL UP FROM WIND FORCES TEARING FASTENERS FREE AS THE FLIGHT PROGRESSED. PILOT PERFORMED A PRECAUTIONARY LANDING SAFELY WITH NO ISSUES AND THE COWLING WAS REPLACED WITH AN UPPER AND LOWER MATCHED SERVICEABLE SET.
RIGHT GEAR RETRACT ROD BENT, ON A WET WINTER DAY THE PILOT PUT THE GEAR HANDLE DOWN AND THE BREAKER POPPED. HE MANUALLY EXTENDED THE GEAR WITH THE HAND CRANK AND GOT 2 GREEN LIGHTS. 3 GEAR COULD BE SEEN FROM THE GROUND AND HE LANDED. LANDING WAS OK, BUT ON ROLL OUT THE RIGHT GEAR COLLAPSED. LIKELY CAUSE IS UPLOCK CABLE FREEZING AND PREVENTING THE SPRING FROM RELEASING THE HOOK WHICH TURNED FREELY ON INSPECTION AFTER THE FACT.
UPON LANDING, LEFT MAIN TUBE WENT FLAT.
THE BRACE ASSY, LT MLG CRACKED AND TWISTED TO ALLOW THE LT MAIN GEAR STRUT AND WHEEL ASSY TO COLLAPSE INWARDS AND UP THROUGH THE INNER GEAR DOOR (WHICH WAS CLOSED) INTO THE WHEEL WELL DURING ROLL OUT AFTER LANDING. ALSO DAMAGED AS A RESULT WERE THE LT FLAP, LEFT AILERON, WING TIP, OUTER RIBS OF THE WING, OTBD UPPER AND LOWER SKINS, VALVE STEM TORN OUT OF THE TUBE, INNER GEAR DOOR, AND MAIN GEAR RETRACT ROD.
CAST SHOULDER THAT HOLDS THE WHEEL BRG RACE FROM MOVING INWARD ON THE AXLE, BROKE OUT, CAUSING THE RACE AND BRG TO TRAVEL INWARD. THE WHEEL WAS NO LONGER HELD CENTERED ON AXLE, AND WHEEL SUBSEQUENTLY HAD EXTREME CAMBER ANGLE. THE EXTREME ANGLE BROKE A PORTION OF THE OUTER SNAP RING GROOVE. THE SUDDEN CHANGE IN CAMBER AND LOSS OF BRG ACTION CAUSE WING TIP TO IMPACT SURFACE. THIS OCCURRED AFTER A COMPLETED LANDING AND ROLL-OUT. ACFT WAS INITIATING A TURN TO POSITION ACFT ON THE GROUND. THE FAILED PORTION OF THE WHEEL APPEARS TO BE .5 THE THICKNESS OF A 40-75D WHEEL.
THE NUT ASSY SEEMS TO HAVE SHEARED WHEN GEAR WAS RETRACTED. POSSIBLE OVERTRAVEL IN UP DIRECTION IS SUSPECTED. POSSIBLE FAILURE OF MOTOR CONTROLLER. WHEN GEAR WAS SELECTED DOWN NOTHING HAPPENED. MAIN GEAR ONLY WAS EXTENDED MANUALLY AFTER MUCH EFFORT BECAUSE GEAR SEEMED TO BE JAMMED UP. 60 AMP BREAKER UNDER FLOOR FOR GEAR MOTOR WAS FOUND POPPED AFTER LANDING. (K)
PILOT NOTED ROUGH RUNNING ENGINE AND MADE PRECAUTIONARY LANDING. LATER INSPECTION REVEALED NR 1 CYLINDER CRACKED AND A HOLE BLOWN THROUGH THE EXHAUST SIDE.
CYLINDER INSTALLED 5/28/98, PLACED IN SERVICE 6/29/98. FOUND CRACK FROM SPARK PLUG HOLE AROUND INTAKE SIDE OF CYLINDER.
BOTH INBOARD AND OUTBOARD AILERON HINGE BRACKET ASSYS ON THE RIGHT WING WERE CRACKED ON THE BOTTOM AND HAD LOOSE OF SHEARED RIVETS WHERE THEY ATTACH TO THE REAR SPAR. THIS DAMAGE CANNOT BE SEEN WITHOUT REMOVING THE FALSE SPAR, BUT CAN BE DETECTED BY LOOSENESS OF THE BRACKET UP AND DOWN.
