AIRCRAFT DEPARTED KBDL AT 0900 LOCAL TIME. DURING LANDING GEAR EXTENSION ON APPROACH TO KHPN CREW GOT A NR 1 BRAKE OVERHEAT INDICATION. AIRCRAFT RETURNED TO KBDL AND MADE AN UNEVENTFUL LANDING. THE NR 1 BRAKE TEMP SENSOR WAS REPLACED AND THE AIRCRAFT RETURNED TO SERVICE.
(CAN) EXPERIENCED A CRACK IN THE RT WINDSHIELD. THE FLIGHT CREW REPORTED THAT THE INITIAL CRACK OCCURRED IN FLIGHT AND WAS A SINGLE CRACK ALONG THE CENTER POST AND BOTTOM. THE FLIGHT CREW REPORTED THAT THE WINDSHIELD HEAT FAILED AT THE SAME TIME. SUBSEQUENTLY, SEVERAL OTHER CRACKS DEVELOPED SURROUNDING THE OUTSIDE EDGE OF THE WINDOW. EXAMINATION OF THE INTIAL CRACK SHOWED THAT IT ALSO HAD A BURN MARK IN IT. THERE WERE NO INJURIES REPORTED. THE WINDSHIELD HAS BEEN REPLACED.
12 MINUTES AFTER DEPARTURE THE PILOT'S WINDSHIELD FACE PLY CRACKED IN SEVERAL PLACES. THE AIRCRAFT RETRUNED TO DEPARTURE AND MADE AN UNEVENTFUL LANDING. THE WINDSHIELD WAS REPLACED AND THE AIRCRAFT RETURNED TO SERVICE.
AIRCRAFT ESS BUSS FILED, MESSAGE DISPLAYED ON ED NR 1, TROUBLESHOT PROBLEM TO AN INTERMITTENT AC ESS BUSS SENSE RELAY (K1XD) IN JB 18. REPLACED RELAY (K1XD), OPS TEST AC ESS BUSS SYSTEM, NO FAULTS NOTED. NOTE: ESS BUSS DID NOT FAIL, SENSE RELAY FAILED GIVING AN INDICATION OF A FAILED ESS BUSS WAS ALWAYS POWERED.
MAIN ENTRANCE DOOR SUPPORT FOOT COLLAPSED. REPLACED MAIN DOOR ACTUATING ROD AND FOOT SUPPORT FOAM. OPS CHECKED MAIN DOOR NORMAL, NO FAULTS NOTED. NOTE: MAIN DOOR FOOT SUPPORT FOAM WAS MISSING CAUSING FOOT TO STICK IN POCKET, CAUSING ROD TO BREAK.
WHILE AT CRUISE, THE CREW OBSERVED NR 2 HYD SYSTEM QTY WAS LOW. CREW MONITORED NR 2 HYD SYSTEM QUANTITY AND TEMPERATURE, HYD TEMP ROSE ABOVE 135 DEGREES CELSIUS AND QUANTITY DEPLETED TO ZERO. CREW SHUT DOWN NR 2 ENGINE PER CNDAIR ''QUICK REF HANDBOOK'' ABNORMAL PROCEDURES SECT 10. CREW DIVERTED A/C TO IRELAND AND LANDED WITHOUT INCIDENT. UPON INSPECTION OF NR 2 HYD SYSTEM, FOUND RT ENGINE DRIVEN HYD PUMP LEAKING HYD FLUID FROM ENGIEN GANG DRAIN. REPLACED NR 2 ENGINE DRIVEN HYD PUMP AND CASE DRAIN FILTER IAW A/C MM. A/C RETURNED TO SERVICE. (M)
AFTER TAXI AND ENGINE SHUT DOWN WITH APU OPERATING, FLIGHT CREW NOTED STRONG ODOR FROM CABIN AREA. FURTHER INVESTIGATION REVEALED ODOR EMANATING FROM LAVATORY VANITY AREA. CREW IMMEDIATELY REMOVED ELECTRICAL POWER FROM AIRCRAFT. AIRCRAFT WAS SENT TO MAINTENANCE FACILITY FOR DETAILED INSPECTION. UPON VANITY ACCESS, PARTIAL DISASSEMBLY AND INSPECTION POTABLE WATER HEATER BLANKET SEVERELY OVERHEATED. ALSO, NOTED WAS THE INTERNAL BLANKET WIRES WERE CHAFED BY CLAMP THAT SECURES HEATING BLANKET TANK ASSEMBLY TO STRUCTURE. MINOR REPAIRS WERE MADE TO DAMAGED INSULATION ADJACENT TO HEATING BLANKET. SYSTEM DE-ACTIVATED PER MEL AND RELEASED.
HPN - NORMAL TAKEOFF EN ROUTE BAHAMAS. APPROX 1 HR EN ROUTE AT FL 310, PILOT NOTED NR 1 ENG VIBRATION INCREASE FROM .3 M/S TO .6 M/S AND INCREASE IN ENG NOISE. ALL OTHER ENG INDICATIONS WITHIN NORMAL PARAMETERS. ENG NOISE SUBSIDED THEN INCREASE IN ENG VIBRATION FROM .6 M/S TO .9 M/S, WITHIN NORMAL LIMITS. PILOT DISCONTINUED FLT, PRECAUTIONARY LANDING AT ISP. BORESCOPE INSP REVEALED NR 2 TURB DISC DAMAGE. ENG REMOVED AND REPLACED. UPON ENG DISASSY AND REPAIR, ALLIED SIGNAL NOTED A GAS PRODUCER SPACER WAS INSTALLED. SPACER CONFIG IS NOT COMPATIBLE WITH ALF502L-2C ENGINES AND THEY SUSPECT ENG DAMAGE A DIRECT RESULT OF INSTALL OF INCORRECT GAS PRODUCER SPACER CONFIG. ENG REPAIRED AND RETURNED TO SERVICE.
